I am the Town Manager of the Town of Apex and pursuant to CFR 122.22 and Section 6(a) of NC DEQ General
Permit No, NCGO10000 I am the officer capable of signing all applications, reports, or other information submitted
to NC DEC, Division of Energy, Mineral and Land Resources on behalf of the town . Both CFR 122.22 and General
Permit No. NCG010000 permit all reports required by the general permit to be signed by a duly authorized
representative of the authorized signatory, if that individual is authorized in writing and it is submitted to the
Director of DEQ. The purpose of this letter is authorize such a representative for myself for this purpose. Please
accept the statement below as the authorization of the Town of Apex Water Resources Director to sign said
documents. The Water Resources Director is responsible for the overall operation of regulated town facilities and
activities and has overall responsibility for environmental matters for the Town as required by Section 6(b) of
General Permit No, NCG010000.
I hereby authorize the Town of Apex Water Resources Director to sign all reports required by the general permit
and other information requested or required by the Director of DEQ and any other documents required by the
general permit for the construction of Town utilities, including the NCG01 Notice of Intent Certification Form ,
