I, Victor A. D'Amato, as a duly registered Professional Engineer in the State of North Carolina, having been
authorized to observe periodically the construction of the project,Summerfield Renaissance Shopping
Center Wastewater System Expansion in Summerfield, Guilford County, for the Permittee hereby state that,
to the best of my abilities,due care and diligence was used in the observation of the construction such that
the construction was observed to be built within substantial compliance and intent of this permit, the
approved plans and specifications, and other supporting materials.

As-Built System Description
In accordance with the approved plans and specifications(dated January 3,2008). Field soil moisture sensors as
indicated in September 12,2008 permit have not been installed.These were not part of the approved engineering
design and Engineer hereby certifies that the system will operate properly in their absence(note that rain sensors
are provided). As-built plan and cross-section of the wet weather storage pond is attached. All other system
elements were installed in substantial accordance with the approved plans and specifications.
