Explanation:
Team industries was not in Significant Non --Compliance (SNC) anytime during 2019. Forthe period of
January —June 2019, Team Industries had no violations. For the period of July— December 2019, Team
Industries had NOVs for pH but was not in SNC.
On March 25, 2019, the Town of Andrews did a pretreatment inspection of Team Industries and found
the plant and processes to be in good shape.
Sampling was performed by the Town of Andrews contract lab Earth Environmental Services on 3/26 &
27/19 and then again on 6/20 & 21/19. Sampling for the SIU was performed by Pace Analytical on the
week of 6/18-21/19.
On receiving the Semiannual Wastewater report from Jennifer Verde (Anchor QEA) in July 2019, it was
discovered that 2 of the BODs performed by Pace Analytical were above the limits set for the SIU but
were flagged with QC errors that indicated that the samples were not read on day 5 for the test, but
instead were read on day 6. BOD standard method SM 5210 B-2011 (Aqueos) requires the samples be
read out on day 5. BODs were also tested by Earth Environmental Services during the same days in
question with results that fell in compliance with the limits for the SIU_ The results provided by Pace
Analytical were determined to be invalid and no NOVs were written.
Sampling was performed for the Town of Andrews by Earth Environmental Services on 10/15 & 16/19
and then again on 11/12 & 13/19. Sampling for the SIU was performed by Pace Analytical on the week
of 12/10-13/19.
Team Industries was found to be out of compliance for pH on 10/15/19, 10/16/19 and 11/12/19 and
BOD on 11/12/19. Terry Yonce (TEAM Industries) and Jennifer Verde (Anchor QEQ) were notified at
each instance of the pH being out of compliance. An NOV was sent to TEAM Industries on February 24,
2020.
On March 5, 2020 TEAM responded to the NOV. The parts rinse tank and the Imprex system had not
been drained in several weeks prior to the occurrences and the pH in the rinse water of those was found
to be low. TEAM has subsequently increased the exchange rate of the rinse tank and the pH has been
stabilized. Fallow up testing provided by Pace Analytical in December 12/9-12/19 resulted in pH results
between 6.3 to 7.8. The cause of the high BOD on 11/12/19 was not addressed.

All parameters were found to be in compliance during the December 2019 testing period.
Team Industries, The Town of Andrews WWTP Personnel, Anchor QEA and Earth Environmental Services
are working together maintain steps need to keep all pollutants beneath permit limits and to bring
Team Industries Andrews, Inc. back into compliance.

Dear Mr. Yonce
Team Industries Andrews, Inc. has been found to be in violation of Part I., for their Wastewater
Discharge Permit (#0004) issued by the Town of Andrews for the Determination Period of 71112019 -
12131/2019. The violations are summarized on the following page. Please note that the industry is
NOT in Significant Non -Compliance for any parameters.
A report containing the probable cause of the violations and plans to return to compliance must be
submitted to this office within 30 days of receipt of this notice, Team Industries Andrews, Itic. will be
included in the Public Notice of Significant Non -Compliance as required by NCAC 02H
,0908 and 40 CFR 403.8 (1).
If Team Industries fails to return to compliance, then additional actions, including increased penalties
or issuance of an enforceable Order, will be completed in accordance with the City's Enforcement
Response Plan. Please be aware that the City's Sewer Use Ordinance gives the Director the
authority to assess penalties of up to $25,000 per day per violation.
