The Division may require modification or revocation and reissuance of the Certificate of Coverage. This permit expires on November 30, 2003. The Division staff is currently in the process of renewing this permit and is scheduled to have the permit reissued by fall of 2003. Once the permit is reissued, the above referenced facility would be eligible for continued coverage under the reissued permit. If you wish for the above referenced facility to be covered under the reissued permit, you must apply to the Division of Water Quality (DWQ) for renewal of permit coverage. Enclosed you will find a Permit Coverage Renewal Application Form. The application must be completed and returned by June 3, 2003. Failure to apply for coverage under the reissued NCG220000 may result in a civil assessment of at least $250.00 and/or a lapse in permit coverage for the above referenced facility. Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from the above referenced facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could result in assessments of civil penalties of up to $10,000 per day. Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial facilities in any of the 11 categories of "storm water discharges associated with industrial activity," (except construction activities). If you feel the above referenced facility can certify a condition of ,.no exposure", i.e. the facility industrial materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. They are as follows: OUTFALL #1: Drains the parking area and log crane operating pile. Stormwater flows to the south side of operating pile in to detention pond #1. (Approx. = 4 acres) OUTFALL #2: Drains the south end of mill site that includes the following: mill line, chip storage area, temporary log storage, grass slopes, and wooded area. (Approx. = 12 acres) OUTFALL#3 Drains grassy and wooded area at the north end of the site. This outfall also drains temporary log storage area. (Approx. = 10 acres) The temporary log storage areas are identical on the north (outfall #3) and the south (outfall #2) ends of the mill site. 1 ask that your department grant representative outfall status to outfall #3 because the stormwater discharge should be identical in respect to the temporary log storage areas. This would require me to monitor only outfalls #1 and outfall #2 in accordance with general permit #NCG220000.
