In accordance with your application for a discharge permit received on September 06,
2016, we are forwarding herewith the subject certificate of coverage to discharge under the
subject state — NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
Should the frequency of wood preservation increase beyond the occasional operation as
described in the permit, please notify the central office at the below number.
If any parts, measurement frequencies, or sampling requirements contained in this permit
are unacceptable to you, you have the right to request an individual permit by submitting an
individual permit application. Unless such demand is made, this certificate of coverage shall be
final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Energy, Mineral and Land Resources. The Division of Energy, Mineral and Land
Resources may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Energy, Mineral and Land Resources or permits required by the Division of
Water Resources, Coastal Area Management Act or any other federal or local governmental
permit that may be required.

Dear Mr. Shields:
Staff from the Department of Environmental Quality (DEQ) conducted a compliance inspection
on June 10, 2016 of the subject facility as part of a multimedia program inspection. We appreciate
your assistance during the inspection. Based on the information you provided on your process and
products and Standard Industrial Classification (SIC) code 2421, along with exposure of industrial
activities to precipitation, and several discrete stormwater conveyances hydraulically connected to
surface waters of the State, the stormwater discharge outfall locations at your facility are deemed
subject to NPDES industrial stormwater regulations.
In accordance with North Carolina General Statute 143-215.1, other lawful standards and
regulations promulgated and adopted by the North Carolina Environmental Management
Commission and the Federal Water Pollution Control Act, all owners and operators of stormwater
point source discharges associated with establishments primarily engaged in activities classified
as Lumber and Wood Products, Except Furniture (SIC 24), are required to obtain an NPDES permit
to cover stormwater discharges from their sites.
Enclosed with this Notice, please find a National Pollutant Discharge Elimination System
application for coverage under General Permit NCG21000O (Notice of Intent) and Technical
Bulletin for General Stormwater Permit NCG210000. Your application for coverage should be

M DIRECTIONS TO FACILITY: From RRO, take I-440 W to US 1 S to Hwy 64 W to Pittsboio. Take Hwy 902 W to Main
St. in Goldston. Turn left on Main St., Goldston Lumber, Inc. is 0.5 miles on the right.
(II) FACILITY DESCRIPTION: The facility saws and planes lumber for pallet cants. Goldston Lumber, Inc. currently employs
9 people who work one 40-hour shift. Inspectors should wear steel -toed shoes and have hearing protection to enter this facility.
(IIn INSPECTION SUMMARY: On June 10, 2016, I (Cheng Zhang) met with Mr. Paul Shields, President/Owner, for a
compliance inspection. There is an office on site where the permit and cyclone records are kept. After reviewing the permit
and the records, we toured the chipper cyclone and the shaving house. All sources were in operation at the time of the
inspection, except that the drill press was taken offline for repair.
(IV) PERMITTED EMISSION SOURCES: Goldston Lumber, Inc. operates under Air Permit Number 07640R04, issued July
24, 2013 and expires June 30, 2018. Under this permit, the facility is authorized to construct and operate the air emissions
sources and/or air cleaning devices and appurtenances described below:

(V) SPECIFIC PERMIT CONDITIONS:
A.1. The Permittee shall comply with Title 15A NCAC, Subchapter 2D .0202, 2D .0512, 2D .0521, 2D .0535, 2D .0540,
and 2D .0611 and 2D .1806.

shall submit the air pollution emission inventory report in accordance with 15A NCAC 2D .0202, pursuant to N.C. General

A.3. PARTICULATE CONTROL REOUIREMENT - As required by 15A NCAC 2D .0512 "Particulates from Wood
Products Finishing Plants," particulate matter caused by the working, sanding, or finishing of wood without providing , as a
minimum for its collection, adequate duct work and properly designed collectors.
In compliance. The shaving house and cyclone are working properly.
A.4. VISIDLE EMISSIONS CONTROL REOUIREMENT -As required by 15A NCAC 2D .0521, visible emissions from
the emission sources, manufactured after July 1, 1971, shall not be > 20% opacity when averaged over a six -minute period.
Appears in compliance — no visible emissions were observed from the emission point during the inspection.
A.5. NOTIFICATION REQUIREMENT - As required by 15A NCAC 2D .0535, the Permittee of a source of excess
emissions that last for > 4 bra that results from a malfunction, a breakdown of process or control equipment or any other
abnormal conditions, shall notify DAQ by the next business day.
Appears in compliance — Mr. Shields indicated that no such occurrences have happened.
A.6. FUGITIVE DUST CONTROL REQUIREMENT - As required by 15A NCAC 2D .0540 "Particulates from Fugitive
Dust Emission Sources," the Permittee shall not cause or allow fugitive dust emissions to cause or contribute to substantive
complaints or excess visible emissions beyond the property boundary.

complaints are on rile against Goldston Lumber, Inc.
A.7. CYCLONE REQUIREMENTS — As required by 15A NCAC 2D .0611, particulate matter emissions shall be
controlled as described in the permitted equipment list.

The wood kiln and boiler have not operated since 2005 according to Mr. Shields.
(Vill COMPLIANCE HISTORY: A review of I -BEAM revealed a violation by this facility in 2008 for 15 NCAC 2D .0202 and
2Q .0304. In 2012, the facility was in violation of 15 NCAC 213.0512 and General Conditions B6 "operating while not properly
maintained" and B 14 "Permittee could not present an air permit upon request."
(VIM 112 r APPLICABILITY: Goldston Lumber. Inc. is not subject to 112 (r).
(IX) STACK TEST OBSERVATION: No stack tests have been performed during the past five years.
CONCLUSIONS/RECOMMENDATIONS: At the time of this inspection, it appears that Goldston Lumber, Inc.
was in compliance with its air permit requirements. This facility should be inspected again in two years.

On August 23, 2007, I received your response to the letter I sent you on July 23, 2007. The initial reason for
this was to determine whether the facility needs a stormwater permit. After reviewing the detailed description
of the process and final product I spoke with the Division's stormwater permitting section and also looked up
the SIC code on OSHA's website (http://www.osha.gov/pls/imis/sicsearch.html). Based on the activities
described I think the SIC code was assigned incorrectly. Because you store product and raw materials outside
this will require a general stormwater permit (NCG210000). I have attached a copy of the application and
technical bulletin for your review. You may also find additional information on our website
