On May 9, 2012, Thomas Ascenzo of the Raleigh Regional Office (RRO) conducted a compliance inspection
(CEI) of Pallet One Inc. Stormwater program. The assistance of Ms. Jerri Cain and staff, was appreciated as it
facilitated the inspection process. The Basin -Wide Information Management System (BIMS) inspection report is
attached.
The following observations were made:
1. The Pallet One Inc. Company is authorized to discharge stormwater from its facility located at 2340 Ike
Brooks Road, Siler City, NC to receiving waters designated as a UT to Tick Creek. A class C stream in
the Cape Fear River Basin.
The Review of your Stormwater Pollution Prevention Plan revealed the following:
• It was indicated that employee training was being conducted by but no documentation was noted.
• It is recommended that all employees are trained to properly respond to spills.
• It was indicated that no spills took place in a three year period, please document this fact and
include a page inside the SP 3 manual.
• A responsible party list needs to include phone numbers and a list of companies that could be
called for cleanup in case of a large spill from one of the gasoline or diesel tanks. This information
should also be posted in a location for the employees.
3. Qualitative Monitoring Forms for June 2011. No other documents were noted in booklet. Please conduct
qualitative monitoring semi-annually as per permit requirement.
4. Analytical Monitoring was noted for June 2011. Please conduct analytical monitoring as per permit
requirements.
5. Outfalls need to be cleared of vegetation and brush for ease of access and inspection. Please install
signs at each Outfall identifying the locations. Outfalls were not observed at this time due to access
difficulty.
6. Site inspection also revealed stockpiled wooden pallets which appeared to be new and unused. If pallets
become oil soaked or contaminated please properly dispose of them..
7. Stormwater Pollution Prevention Plan (SP3) is required to be reviewed and updated annually,. The
inspection revealed the SP3 had not been updated. North Carolina

8. It was observed in the inspection that in front of the vehicle maintenance building there is oil
stained ground indicating that the equipment is leaking oil onto the ground contaminating the
soil. Please remove all contaminated soil and properly dispose of the soil by hiring a
environmental cleanup company. Implement a procedure to eliminate the problem of oil leakage
by using oil absorbents or bringing leaking vehicles inside the garage for repair. Label old oil
containers with "Used Oil".

June 11, 2012.

Stormwater Pollution Prevention Plan
PalletOne is a pallet manufacturing facility located at 2340 Ike Brooks Road
in Siler City, NC, consisting of four separate buildings, housing an
assembly operation, cut-up operation, repair operation, maintenance
shop and administrative offices.
On site potential pollutant sources which might possibly contribute
to the contamination of stormwater discharges are gasoline, #2 diesel
fuel, motor oils, lubricants paints and inks.
These possible pollutants are stored in OSHA approved containers
and Material Safety Data Sheets are on file for all of these products.
Gasoline and Diesel fuel are stored in 500-gallon contained tanks.
Any spillage of possible contaminates are contained by the use of
sawdust, containers of which are readily available in every building.
Spills are then immediately transferred to trailers and are burned for
fuel at local brick plants. All containers are stored in secured areas
with proper labels. Periodic inspections for evidence of wear or damage

to containers are a part of regular safety inspections.
With regard to the movement of containers, whenever possible only
empty containers are moved. In the event of the moving of a full or
partially full container, we first make sure the container is sealed and properly
secured on whatever form of conveyance is being used. We then remove any
obstacles in the path of travel that would impede the container or cause leaks or
ruptures. We also have a back-up container available in the event of a spill during
any move.
Consideration is given to storage locations to minimize the possibility for stored
products coming into contact with stormwater. Storage containers are
stored in a dry environment or placed within spill barriers.
We have had no significant spills or leaks of pollutants within the past three years.
I, Gary Lawing, Plant Manager, certify that I have evaluated the two outfalls for the
presence of non-stormwater discharges.

The physical lay -out of our facility was designed with the intent of diverting the
stormwater runoff away from areas of potential contamination.
To prevent leaks and spills from bulk storage areas for the gasoline and #2 diesel
fuel (500-gallon tanks), we have constructed a tank containment enclosure with a
capacity of 1000 gallons. All oils are stored in double -walled tanks and we have a
supply of 250 gallons on hand at any given time. Our lubricants are stored in
double- walled tanks and we have an average on -hand quantity of 250
gallons. Paint,used in our assembly department, is stored in that department in
resealable containers with an on- hand quantity not exceeding 20 gallons.
Ink, used for identifying customer specific pallets, is also stored in our
assembly building in resealable containers with on -hand quantities of
approximately three gallons.
During the routine servicing of equipment we lay a sawdust perimeter around
the affected area during the oil changing process.

We have implemented the use of vegetative filter strips and graveled areas
for filtering the stormwater runoff.
Good housekeeping practices with specific documented schedules of preventive
maintenance on all plant equipment and systems is an intergral part of our regular
safety program. Inspection of storage areas and containers is also a part of our
regular safety inspections.
Spill containment procedures are a part of our regular safety training for all
employees and regular preventive maintenance is part of the written job
description for all personnel involved in any of the facility's operations
that have the potential to contaminate stormwater runoff. That training is the
responsibility of each department supervisor and is coordinated with the
safety director.

Two members of our Maintenance staff, David Gwyn and Steven Howard,
having had extensive training in hazardous material handling and control,
serve as our Emergency Spill Response Team leaders. They are supported by
departmental supervisors.

On February 20, 2007, Ms. Stephanie Brixey of the Raleigh Regional Office conducted a stormwater evaluation
inspection of Pallet One with the assistance of Ms. Jerri Cain and yourself. All help was appreciated as it eased the
inspection process.
The following observations were made:
1. The facility does not have a Stormwater Pollution Prevention Plan as required by the General Permit
NCG210000.
2. The facility is responsible for conducting qualitative monitoring semi-annually and analytical monitoring annually
which has not been completed as required by the permit. This monitoring must begin immediately. Please submit
your results with the SPPP.
3. A copy of the permit and Certificate of Coverage was noted as being on -site.
Within 60 days of your receipt of this letter, please provide a copy of the Stormwater Pollution Prevention Plan
(SPPP). Please submit your response to the following:

I have reviewed the Stormwater Pollution Prevention Plan that you submitted. The plan was received March 06, 2007.
The plan covers most of the requirements of the General Pen -nit No. 210000. After reviewing the plan I noted the
following:
1. A responsible party list needs to be included with phone numbers in the plan. This information should also be
posted for employees at the facility. I would also recommend having a list of companies that could be called for
cleanup in case of a large spill from one of the gasoline or diesel tanks.
2. Include in the plan the inspection schedule (weekly, monthly, etc.) and a copy of the forms used to do the
inspections.
3. Make sure employees are trained at a minimum annually and keep this documentation on file for review. The
facility personnel (or team) responsible for implementing the training must be identified in the plan.
4. Make sure the USGS map and site map drawn to scale with distance legend indicating locations of industrial
activity are on -site.
5. There needs to be a certification statement signed as required in the general permit in Section A Part 1(e). This
should be included in the plan. .
6. Continue to do the analytical and qualitative monitoring as required by the permit.
7. Keep a copy of the general permit and certificate of coverage with the plan.

Your facility is currently covered for Storntwater discharge under General Permit NCG21 W00. This permit expires
on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by Carly spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier. we arc informim_ you in

In response to your renewal application for continued coverage under the General Permit
NCGO40000, the Division of Water Quality (DWQ) is forwarding herewith the General Permit
NCG210000 which is a new General Permit to cover most of the Timber Products Industry. You
have now been given coverage under NCG210000 and your coverage under NCGO40000 is hereby
terminated. This permit is issued pursuant to the requirements of North Carolina General Statute
143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental
Protection Agency dated December 6, 1983.
The following information is included with your permit package:
■ A copy of the stormwater general permit NCG210000.
■ A new Certificate of Coverage under general permit NCG210000.
■ A Stormwater Pollution Prevention Plan Certification Form. This form certifies that you have
developed and implemented the Stormwater Pollution Prevention Plan (SPPP) required in your
permit (both NCGO40000 and NCG210000). This form must be completed and returned to the
Division within 30 days of receipt of this letter. DO NOT send the SPPP with the signed form.
■ Five copies of Analytical Monitoring forms.
■ Five copies of Qualitative Monitoring forms.
■ A copy of a Technical Bulletin on the stormwater program which outlines program components
and addresses frequently asked questions.
Your certificate of coverage is not transferable except after notice to DWQ. The Division of Water
Quality may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
DWQ or permits required by the Division of Land Resources, Division of Air Quality, Coastal Area
Management Act or any other Federal or Local governmental permits that may be required.
