On May 10, 2011, Dave Parnell of the Raleigh Regional Office carried out a stormwater compliance
evaluation inspection (CEI) for the above permit. Mr. Mark Withrow, Compliance Manager, was
available to assist Mr. Parnell. A file review indicates that this was the first time DWQ had inspected
this facility. See the enclosed inspection checklist. A number of issues emerged and are discussed
below:
1. This permit was issued September 15, 2008 and it expires July 31, 2013. The permit was
available during the CEI.
2. Mr. Withrow had possession of the Stormwater Pollution Prevention Plan (SPPP) for this
facility during the inspection. The SPPP was produced in 1995 and has not been reviewed and
updated since that time. There is a good deal of relevant information within the plan, but a
complete re -production of the plan is needed. Mr. Withrow knew the location of the
Stormwater Discharge Outfalls (SDO), but these must be further developed and correctly
located on the detailed site map, within the SPPP. Mr. Withrow could not produce any
monitoring results for 2010. He stated that the mill had been closed for extended periods of
time during the economic downturn. As discussed during the CEI, the NCG210000 Stormwater
Permit requires analytical and qualitative monitoring, be conducted at each (all) designated
SDO, in the frequency directed by tables within your permit.

as they are completed.

the permit reissued by early spring of 2003. Once the permit is reissued. your facility would be eligible for
continuer) coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Warr
Quality (DWQ) for renewal of your permit coverage. To slake this renewal process easier. we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed :md returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff:md budget constraints, letters conlirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified. may result in a civil assessment of at least $250.00.
Largo penalties may be assessed depending on the delinquency of the request. Discharge of stormwater Ilnm your
facility without coverage under a valid stornlwarer NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties olup to $10.000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion' to all operators of industrial

construction activities). If you feel your facility can certify a condition of "no exposure". i.e. the facility industrial
materials and operations are not exposed to stormwater. you can apply for the no exposure exclusion. For additional

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission and the Federal Water Pollution Control Act, as amended, this permit
is hereby issued to all owners or operators, hereafter.permittees, which are covered by this
permit as evidenced by receipt of a Certificate of Coverage by the Environmental Management
Commission to allow the discharge of stormwater to the surface waters of North Carolina or
separate storm sewer systems conveying stormwater to surface waters in accordance with the
terms and conditions set forth herein.
Coverage under this general permit is applicable to all owners or operators of stormwater point
source discharges associated with activities classified as establishments primarily engaged in
Timber Products [standard industrial classification (SIC) 24]; and like activities deemed by
DWQ to be similar in the process and/or the exposure of raw materials, products, by-products, or
waste materials.
The following activities are specifically excluded from coverage under this General Permit:
Wood Kitchen Cabinets [SIC 2434), Wood Preserving [SIC 2491] and Wood Chip Mills.

The General Permit shall become effective on August 1, 2008.
The General Permit shall expire at midnight on July 31, 2013.
