The purpose of this correspondeom is to respond to NCD1• NR correspondence dated May 17, 2012
rewarding a Compliance Evaluation Impaction that was conducttai. at the above referenced site on April
19, 2012 and to summarize our discussion on June 11, 2012. As we discussed, Geological Resources,
Inc. (OR1) is assisting United Salvage & Auto, Inc. (USA) to achieve compliance with the raquirementa
of the Stormwater Permit (No. NCG20046C) and your above referenced eotrrspondence.
As you correctly noted, a Stormareter Pollution Prevention Plan (SWPPP) has been in place at the site.
However, some elements of the plan are in the pmoeas of being implemented. During the development of
the SWPPP, it was determined that a combination of earthen beau along the southern property boundary,
two rip rap lined di v and a sediment basin would be required to efficiently manage any stornlvrater
that may be gaocat�thc cite The purpose of the rip rap lined diversions is to divert stormwator
runoff fro® upgradiea areas to the proposed sediment basin. However, subsequent to completion of the
plan, USA determined that, they would need an additional buffer between the shredder area and the
eastern and southern fence line. The additional butler was requested to allow room for vehicular traffic
that serviced the shredder area. As a result, it was determined that the shape of the proposed basin as well
as the exteslt of the diversion ditches that were to feed the basin would need to be reconfigured. USA also
requested GR I to evaluate if the design of t3a proposed basin could be such that a waiver on the analytical
aonitoring fequir=cnts of the Stormwater permit could be requested. Since the timings of the facility
preclude stormwater sampling if the representative stoma event were to occur after hours, a waiver on the
sampling. requirement was investigated
Based oa subsequent conversations with Mr. Robert PattersoA. of the Siormwater Permitting Unit at
NCDENR, DWQ, it was determined that if the basin were designed to capture and hold sediment as well
as stormwater generated by a 25-year storm event; end if the captured water were reused to ensure that the
basin maintained suffidcat capacity between basin water reuse and a subsequent 25-year storm event, a

revised design and configuration of the diversion ditches, before a decision could be made on the waiver
an sampling. 712e NGD&NR also indicated that a review and approval of the design would be required
before 1JSA could proceed with the construction of the revised basin. .
Pending approval from USA, GRI will finalize the redesign of the basin and ditches and submit the
redesign to the Starmwater Permitting Unit. As we discussed, the implementation of ooastruction of the
basin is dependent upon the review and approval by the Stormwatex Permitting Unit USA bas assured
that the construction of the basin will be completed upon approval from the NCD3WR within a reasonable
time frame. All monitozngrequiroments of the permit will commence immediately thereafter.
As required in item 2 of your oorreapondence, USA will:
• Include doctra=Italion of any significant spills within the S R'PPP. Please note that USA has not
Indicated that there have been any significant spills in the recent, past Going forward, any
significant spills will be maintained on file for three yuam
• Conduct employee training for 2012 in ectwrdance with the SWPPP and include documentation
and a sigrntp sheet of the training event. Please note that employee training is conducted on an on-
going basis at the DIeility. Going forward, USA wiU ensure that documentabon of such training
related to the SWPPP and associated procedures wilt be kept on file.
• Maintain an updated list of responsible parties within the SWPPP, Please note that the primary
and secondary contacts have not changed since the SWPPP was put in plecc.
• Direct GRI to update the SWPPP in cOujuactiom with the construction of the sediment basin to
better address mVirements of Part 11 Section A of the stormwaur permit. Any requirements not
dependent upon the construction of the referenced basin will be updated immediately.
Conduct monitoring in accordance with the permit and Pending submittal to NCDENR
Stotmwater Permitting Unit. Reporting procedures and record keeping will be implemented
immediatelY. Inspections of sedimentation and erosion control structures already in place at the
facility will ooannmnee inunedintoly and documentation will be maintainm on file,
SPeciHcally related to item 4 of your correspondence,
• The SWPPP will be updated as required and NCD&NR will be informed as soon as the updates
are implemc sled. Please note that the SWPPP will be updated within three woeh of finalization

diversion ditches.
• As .we discussed, the Proposed godiment basin will )rake one SDO approximately located at the
southeast corner of the facility. Monitoring of this SDO in accordance with the parrmt will
oommemce immediately following Construction of the basic Qualitative monitoring of any
temporary SDO in this general location will commence immediately. Going forward, upon
comrpl'tion of construction of the basic, USA will comply with monitoring requirementa of the
designated SDO.

On April 19, 2012, Dave Parnell of the Raleigh Regional Office carried out a stormwater compliance
evaluation inspection (CEI) for the above permit. Mr. Scott Sawyer was available to assist Mr. Parnell.
A file review indicates that this was the first time DWQ had inspected this facility. See the enclosed
inspection checklist. A number of issues emerged and are discussed below:
The current permit was issued October 10, 2008 and will expire 5 years from that date. The
permit was produced during the CEI.
2. The Stormwater Pollution Prevention Plan (SPPP) was reviewed during the inspection. The
plan was reviewed and updated within the last year, but still contained some areas that are in
need of attention:
a) A list of significant spills is required to be contained within the SPPP. This list must extend
for the last three years.
b) Employee training is required on an annual basis and documentation is required to be within
the SPP.
c) Maintain an updated list of responsible parties within the SPPP.
d) Reformat the SPP to better address the requirements of Part lI Section A of your stormwater
permit.

Mr. Sawyer could not produce any monitoring results. As discussed during the CEI, the permit
requires qualitative monitoring and analytical monitoring (in addition to the required
vehicle maintenance components of the analysis) be conducted at each (all) designated
Stormwater Discharge Outfalls (SDO), in a frequency directed by tables within your permit.
4. Please prepare a response and timeline addressing the following:
A) Re -produce an updated Stormwater Pollution Prevention Plan adhering to all
components of your Stormwater Permit.
B) Implement the monitoring requirements of your permit as soon as the SDO(s) have
been verified by your engineer. Provide documentation of the monitoring results to this
office as soon as they are completed.
C) Provide a map identifying the location of your SDO.
Please provide a written response to this letter, within 30 days of the receipt of this letter, detailing
United Salvage and Auto's compliance efforts with respect to the bold items found above in #4A, 9413,
and 44C. If there are any questions or comments about this inspection, please contact Dave Parnell at

general permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-
