On June 9, 2014, the Division of Energy, Mineral and Land Resources received your request to rescind your
coverage under Certificate of Coverage Number NCG180202. In accordance with your request, Certificate
of Coverage Number NCG 180202 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to waters
of the State without valid coverage under an NPDES permit is against federal and state laws and could result
in fines. If something changes and your facility would again require stormwater or wastewater discharge
permit coverage, you should notify this office immediately. We will be happy to assist you in assuring the
proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would inform the
new or prospective owners of their potential need for NPDES permit coverage.

1. Ron Boone, of the NC Division of Water Quality (DWQ), Raleigh Regional Office (PRO), Surface Water
Protection (SWP) section, conducted a stormwater compliance evaluation inspection (CEI) of the subject
facility on 8 November 2007. The CEI was conducted to assess Hon's compliance with their general
stormwater discharge permit, NCG180202. Your assistance and cooperation was greatly appreciated
during the inspection. The results of the CEI are summarized below and in the attached inspection report.
2. The facility is located at 7966 NC 56 Highway, in Louisburg, Franklin County, NC. The NCGIS0202
permit/Certificate of Coverage (COC) allows the discharge of stormwater from point source discharges
associated with industrial activities classified as establishments primarily engaged in the manufacture of
furniture and fixtures [standard industrial classification (SIC) 25], to an unnamed tributary of Wolfpen
Branch, which is currently classified as a class C, nutrient sensitive water (NSW) in the Tar Pamlico river
basin.
3. The stormwater pollution prevention plan (SP3) for the facility, as required by Part II, Section A of the
permit was reviewed and found to meet most requirements. The plan does contain all the major
components and has been fully implemented. There were some minor discrepancies as noted below:
A. The site map shows six stormwater discharge outfalls (SDO), but one has since been eliminated.
Please update the plan to reflect this change.
B. The general location map does not include accurate latitudes/longitudes for each of the SDOs. Please
update the map to show the latitudes/longitudes for each of the active SDOs. If you cannot fit them on
the general location map they can be included in the text of the SP3 but their location should be
referenced on the general location map.
C. Loading/unloading of chemicals currently takes place at loading dock 2.21a, as it is identified on the
site map. However, this is not a best management practice (BMP) written into the SP3. It is suggested
that you add it as a BMP to the SP3 and train your employees on it to ensure the loading/unloading of

chemicals takes place only at this loading dock because it does not have a drain that drains liquids to a
stormwater conveyance like other docks at the facility.
D. If you go a year without any spills, it is suggested that you make note of this in the SP3, perhaps
during the annual review, and date the entry. This clears up any ambiguity as to whether any spills
have occurred since the last inspection.
E. The annual review of the SP3 must be signed and dated.
4. The site was generally clean and well maintained. All five SDOs were inspected and no concerns were
noted. —The secondary containment for the hazardous waste storage area contains a drain in the northwest
corner of the floor of the containment. You stated that you thought it was plugged because normally water
will stand in the drain. To be. certain and to prevent releases of potentially hazardous chemicals when
spills occur in the secondary containment, please pour concrete in the drain or otherwise ensure it is
completely sealed.
There was a small amount of unused equipment stored outside exposed to precipitation. It would be better
to either store this equipment inside a building or under removable covers. Alternatively, you may wish to
consider disposing of any unneeded equipment. Such exposure to precipitation can cause contamination
of stormwater runoff. Likewise, all dumpsters on the site should remain covered at all times when not in
use.
6. Qualitative monitoring is being accomplished as required in Part II, Section B of the permit.
Please note that violations of your permit are also violations of NC General Statute 143-215.1 and are
subject to civil penalty assessments. Please reply in writing to items in paragraphs 3 — 5 of this letter,
within 30 days of receiving it. Your response should indicate your intentions to correct the deficiencies
listed above, how you plan to correct the deficiencies and an implementation schedule for those corrective

Comment: Site plan does not reflect accurate lats/longs for each dutfall. Site plan
shows six SDOs but one has been eliminated and site plan should be updated. Suggest
that permittee update BMPs to include that chemical loading and unloading only take
place at dock 2.21a, as it is specified in the SP3. This dock does not have a drain to a
SW conveyance. Permittee needs to update contact numbers for reporting spills to the
state and to spill plan responsible personnel. Also, if no spills occur during the year,
please include in SP3 an annual notation that no spills have occurred in the past twelve
months. Finally, the annual review of the SP3 must be signed and dated.

In response to your renewal application for continued coverage under general permit NCG180000, the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A copy of general stormwater permit NCGI80000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Forth. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin on the Stormwater program which outlines program components and
addresses frequently asked questions
* A corrected Certificate of Coverage if you indicated a name change on the Renewal Application
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law rule, standard, ordinance, order, judgment, or
decree.
