Johnston County
Mitch Hayes of this office performed the subject inspection February 20, 2007. Your assistance and
cooperation with this inspection was greatly appreciated. The inspection was performed for the purpose of
permit issuance. If you have any questions about your permit, please contact Aisha Lau at 919.733.5083
extension 578. Attached is a Basinwide Information Management System (BIMS) inspection checklist
summarizing the inspection. Below is a list of and findings and recommendations developed from the
inspection:
1. The Stormwater Pollution Prevention Plan (SPPP) was implemented June 10, 2005 and updated
October 27, 2006. All requirements in Section A are being met. Qualitative Monitoring will be
required at all stormwater discharge outfalls as outlined in Section B of the SPPP. A Qualitative
Monitoring Form can be found at. the bottom of the page on the website
http://h2o.enr.state.nc.us./su/Forms_Documents.htm Filled out forms need to be retained by the
Permittee for a period of 5 years.
2. A comprehensive Spill Prevention and Stormwater Plan was developed in November 28, 2006 and
contains all permit requirements. Employees are well versed on proper spill response and cleanup
procedures. Record of attendance and the type of training are documented.
3. The facility has three discharging stormwater outfalls, 002, 003, and 004. The facility also has outfall
001 in which process wastewater flows into a storage lagoon. The process wastewater consists of
fibrous (paper) material, dyes and other materials used in the manufacturing process. The fibrous
material is allowed to settle and is pumped out and processed by McGill Environmental. The liquid
phase of the process wastewater is discharged and treated at the Benson Wastewater Treatment Plant
under Industrial Use Permit No. 0001. The East side of the property drains and flows to Outfall 003.
The South side of the property drains and flows to Outfall 002. Outfalls 002 and 003 discharge to
Mingo Swamp off the property. The North side of the facility and parking lots discharge to a large
pond Outfall 004. The pond flows into a small -unnamed tributary, is piped under US Interstate - 95
and flows into another pond. This pond flows into Mingo Swamp, downstream of Outfalls 002 and

003. Water in the pond at Outfall 004 appeared clear. Water in the drainage ditches at Outfalls 002 and
003 appeared clear with no impairments noted.
