On 8/6/2013,the Division of Energy, Mineral and Land Resources received your request to rescind your
coverage under Certificate of Coverage Number NCG 170134. In accordance with your request,
Certificate of Coverage Number NCG170134 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to
waters of the State without valid coverage under an NPDES permit is against federal and state laws and
could result in fines. If something changes and your facility would again require stormwater or
wastewater discharge permit coverage, you should notify this office immediately. We will be happy to
assist you in assuring the proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would inform
the new or prospective owners of their potential need for NPDES permit coverage.

Actually, I hope it will still be me! Seeing as how I am a temp, I am only allowed to stay for 11 months and then
I have to leave for 1 month before I could come back. Chances are pretty good that I will be able to come back.
Keeping my fingers crossed! If I am not back though then I guess you would probably have to touch base with
Bradley Bennett to see who would be taking care of the rescissions then. Thanks!

Hate to hear you're leaving. Hope it's for bigger and better things.
I did my site visit for this one today. Unfortunately, they are not quite ready for rescission. They are still in the process of
removing some scrap metal and pallets from outside, which my contact said should be completed by mid -November. My
plan is to revisit the site in late November and hopefully be able to OK their rescission request at that time.
Do you know who I should contact at that point?

On September 27, 2010, Mark Brantley, Environmental Chemist, from the Fayetteville Regional Office
of the Division of Water Quality, conducted a site inspection at the Gulistan Carpet facility located at 3140 NC
Hwy 5, in Moore County, North Carolina. A copy of the Compliance Inspection Report is enclosed for your
1'eview. Mr. John Jackson was also present during the inspection and his time and assistance is greatly
appreciated. Stormwater from this facility drains to Aberdeen Creek, a Class C stream located in the Lumber
River Basin. The site visit and file review revealed that the subject facility is covered by NPDES Stormwater
,General Permit-NCG 170000.
Accordingly, the following observations were noted during the Division of Water Quality inspection:

In accordance with your request received September 9, 2003, the Division is forwarding the subject
permit modification. This modification documents the change in name/ownership at the subject facility. All other
terms and conditions in the original permit remain unchanged and in full effect. This permit modification is issued
under the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U. S. Environmental Protection Agency dated December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit modification are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days
following receipt of this letter. This request must be a written petition conforming to Chapter 150B of the North
Carolina General Statutes, filed with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh,
North Carolina 27699-6714. Unless such demand is made, this decision shall be final and binding.
This permit does not affect the legal requirement to obtain other permits which may be required by the

Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A copy of general stormwater permit NCG170000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements,
and addresses frequently asked questions

Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

This correspondence is to acknowledge receipt of your request for renewal of the subject facility's stormwater
discharge permit coverage under General Permit NCG170000. The Department appreciates your attentiveness to
this matter. At this time the Department has just begun the renewal process for this general permit.
The Department will, as part of the renewal process, send letters to each permittee notifying them of the actions that
will be required to renew their permit coverage. We anticipate these letters going out within the next few weeks.

the modified Certificate of- Coverage page for the subject facility. The change is in name and.
ownership. All other terms and conditions in the original permit remain unchanged and in full
effect. This Certificate of Coverage is issued pursuant.to the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement betweemNorth Carolina and the
U. S. Environmental Protection Agency dated December,6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this
permit are unacceptable to you, you have the right to request an individual permit by
submitting an individual permit application. Unless such demand is made, this certificate of
coverage shall be final and binding. Please take notice that this certificate of coverage is not
transferable except after notice to the Division of Environmental Management. The Division of
Environmental Management may require modification or revocation and reissuance of the
certificate of coverage.

In compliance with the provision of North Carolina General Statute t43-215.1, other lawful standards and
regulations promulgated and adopted by the North Carolina,Environmental Management Commission, and the
Federal Water Pollution Control Act, as amended,

NC Hwy 5
Aberdeen
Moore County
to receiving waters designated as Abcrd:7en Creek, class C, in the Lumber River Basin
in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III
and IV of General Permit No. NCG 170000 as attached.
This Certificate of Coverage shall become effective January 16, 1996.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
