Dear Mr. Smith:
In response to the Notice of Violation from the site visit to Vernal Paving Inc. on June 4, 2013,
the following actions have been taken:
1. The Stormwater Pollution Prevention Plan has been implemented.
2. Qualitative Monitoring has been conducted.
3. Analytical Monitoring has been conducted.
4. Employee training and documentation of such has been implemented.
5. Spill documentation page has been included in the Spill Prevention Pollution Plan.
6. The Spill Prevention Pollution Plan has had the following co pG ad;
Feasibility alternatives to current practices, best management summary,
stormwater facility inspection program page.
7. vuiiaii ;;i has beeo cleared of vegetation to facilitate access for sampling and visual
inspection.
8. Outfall #2 has been cleaned and rip rap installed.
9. A rain gauge has been installed at the facility.
