In accordance with your application for a discharge permit received on November 3,
2006, and again, revised, on March 20, 2007, we are forwarding herewith the subject certificate
of coverage (COC) to discharge under the subject state — NPDES general permit. This permit is
issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection
Agency dated May 9, 1994 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except upon request
to the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act or any other
federal or local governmental permit that may be required.
- An Authorization to Construct treatment facilities has been issued concurrently with this
COC. The Raleigh Regional Office, telephone number (919) 791-4200, shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in -place
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted facility
has been installed in accordance with the NPDES Permit, this Authorization to Construct, and
the approved plans and specifications. Mail the Certification (attached) to the Stonnwater
Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617.

regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the

to receiving waters designated as unnamed tributary to Swift Creek, a class C NSW water in the Neuse River Basin
in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, 11, III,
IV, V, and VI of General Permit No. NCG 140000 as attached.
This certificate of coverage shall become effective March 21, 2007.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

Dear Mr. Pace:
On November 3, 2006, the Division of Water Quality, Stormwater Permitting Unit, received your
application for an NPDES permit to discharge stormwater and wastewater from the subject site on
New Bethel Church Road in Garner. NorthCarolina rules require DWQ to conduct an engineering
review of your permit application. The submitted information is insufficient for us to conduct that
review, and we are requesting additional information in order to continue processing your
application. Please submit additional information addressing the items listed below.
1.. Site plan. Please consider that as reviewers, we can't tell from the site plan what control
measures you are proposing, and whether or not they are sufficient, because the drawing
does not present a complete picture of the site with respect to our items of interest in a
review. Specifically, we need additional information for the following items: "
a. We must know where the wastewater flows originate - - the areas of the site are not
labeled so that we can identify where wastewater originates. For most ready -mix
plants we need to see the aggregate piles, the truck wash off station, the recycle
system (if one is proposed), the recycle system overflow piping, the batch plant, and
the drainage direction of truck wash off flows and batch plant wash down flows and
aggregate irrigation flows.
b. We must know how the two types of waters on the site (stormwater and wastewater)
are collected, conveyed, treated (wastewater), and discharged. This information is
not presented on the site plan.
c. We must know where all the wastewater and stormwater discharge points are - -
they're not shown on the site plan.
d. We require finish grade topo lines on a to -scale site plan so that we can confirm for
ourselves that the measures you are proposing will indeed capture stormwater runoff
and wastewater from the industrial activity areas of the site.
e. An inset in the site plan should be a location map for the site consisting of a USGS
topo map with the location of the site indicated.
2. Site waters control narrative, design basis, design calculations, and design details. Please
consider that you have applied for authorization to discharge wastewater and stormwater
under General Permit NCG140000. DWQ cannot grant coverage under the general permit
without an understanding of how those discharges will be controlled. :The brief narrative
provided in the submittal is insufficient for us to understand how you will control both
stormwater and wastewater. Specifically, we need information on the following items.

a. On item 8 of the submitted NOI form you have indicated that there will be no
discharges from the site. But my understanding from phone conversations with
McCarthy staff is that discharges of either stormwater or wastewater will take place.
Please correct my understanding or else provide a narrative description of how
stormwater will be collected, segregated from wastewater, and discharged.
b. Further, please'note that the permit contains limitations on pH, TSS, and Settleable
Solids for wastewater discharges. Wastewater discharges could include drainage
from irrigated aggregate piles, truck wash off, truck rinse out, recycle system
overflow, and batch plant hose down. You have indicated on the NOI form that the
only wastewater present will be the truck wash out, and that there will be no
wastewater discharge. Please revisit this determination, and reconfirm or revise it.
If there are additional wastewaters, and if any of these wastewaters leave the site,
you must present calculations under the seal of a PE that predict compliance with the
permit limits. Or, in lieu of predicting compliance with the permit limits, DWQ will
grant coverage under the General Permit if you present water balance calculations by
a PE that establish that there will be no wastewater discharge from the site in
response to the 25-year, 24-hour rain event.
c. The site plan indicates that four basins will be provided for wash out solids. Please
submit engineering sizing calculations, under the seal of a PE, showing the adequacy
of the proposed basins with respect to the quantity of wash out generated.
3. Finally, please note that we need three copies of any re -submittal.
Please call me before your re -submittal so that we can go over verbally your responses to our
request for additional information.
