DIRECTIONS From FRO, take Hwy 401 South toward Raeford. After you cross into Hoke County, take a right at the stoplight at Wayside Road (a PURE gas station is on the left, a BP on the right). Go about 1 '/a mules until road ends, and then turn left at Plank Road (you will be on Ft. Bragg). Drive - 13 miles until you get to Hwy 211. Turn right and go west on Hwy 211 for almost 7 miles. Veer right onto South Street, and go about %z mile until stoplight at US Hwy 1. Continue forward onto Hwy 5 North for - 6 miles, thru Pinehurst, until highway ends at Hwy 211. Turn left, go west on Hwy 211 for almost 1.%x miles, and then turn right at light onto Juniper Lake Road (BP station will be on the right). Go about 1/4 mile and look for driveway with `Ready Mix" sign on left (between Midsouth Drive and Pinesage Drive). II. SAFETY CONSIDERATIONS Standard DAQ FRO safety gear: Be aware of trucks entering, leaving, loading, and unloading. III. FACILITY/PROCESS DESCRIPTION This is a batch concrete mixing facility. Fly ash and cement are trucked into the plant and stored in silos. Sand and gravel are loaded into a bin that conveys it to a holding bin. Measured amounts of sand and aggregate are dumped onto a conveyor belt that feeds directly into a cement truck. Water is added during this entire process. The measured amount of cement and fly ash are loaded into a weigh hopper and dispensed into the mix truck. The materials are mixed while driving to the job site where it is poured. OPENING CONFERENCE On 27 September 2017, 1, Stephen Allen, and Joshua Harris, both of FRO DAQ, and Mark Brantley, FRO DWR, arrived at the facility for a multimedia inspection and met with Jose Andino, Plant Manager. Mr. Brantley was onsite to review the plant's storm water permit. We explained to Mr. Andino that even though his air permit has been rescinded, he may still receive regular air quality inspections, or compliance assurance visits. Mr. Andino verified the FacFinder information, and updated that facility's contact information; those changes have been updated in IBEAM. We provided Mr. Andino a copy of the "Facility Compliance Tracking Checklist" to use if he'd like since his permit has been rescinded as requested under 2Q .0102(d). Mr. Andino stated that no changes have been made to the inspection and maintenance regimen at the facility, and that the control devices are inspected monthly by facility personnel and he provided us with the documentation for each monthly inspection. Mr. Andino told us he is aware of fugitive dust potential for the facility and reminds each driver of the 10 mph speed limit. A sprinkler system has been installed to suppress fugitive dust on the property. During dry conditions a cement truck disperses water around the property to include the haul road. He said there have been no dust complaints received over the past year. He guided us on the facility inspection. VI. CURRENT THROUGHPUT: Throuehouts Operating hours 0700-1700 Monday -Friday Employees 4 including drivers Concrete 2016:8,551 Production (cu yd) 2015: 13,843 2014:13,667 2013: 20,183 2012: 18,169 2011:13,044 VH. INSPECTION WALK-THROUGH: The facility was operating during this inspection. One truck -load was observed but no more occurred after that. • Truck mix operation: This batch plant has a central bag filter system (CD-1) that includes a truck mix load out shroud and vacuum. The cement/fly ash weigh hopper is also connected to this bag filter and controls dust 'while loading the truck. The truck load out ductwork appeared to be in good condition. • Cement/Fly ash silos: I observed no evidence of spills or overfilling. Administrative controls are used to keep track of cement usage. I did not observe a flyash silo load. • Bag filter system: The bag filter housing appeared to be in good condition; it connects to silos, the - r- - truck load out,: and cement/fly ash weigh hopper: The ductwork and flex hose connections also appeared to be in good condition and no visible holes were noticed; the ductwork appeared in good condition. • Fly Ash/Cement Weigh Hopper: During the cement truck load out light dust emissions were observed for about 30 seconds and dissipated to zero. The emissions dissipated before crossing the property line. VIII. APPLICABLE AIR QUALITY REGULATIONS: A. 15A NCAC 2D .0515 — PARTICULATES FROM MISC. INDUSTRIAL PROCESSES — Particulate emissions shall not exceed allowable emission rates as calculated by the following equations: E = 4.10 * (P) 0.67 for P < 30 tons/hr, or E = 55 * (P) ° 1 1 - 40 for P >30 tons/hr APPEARF,D IN COMPLIANCE: These calculations were made when the facility was permitted, and the previous permit reviews indicated that that facility, when controls are properly maintained and operated, would not exceed these limits. The facility's particulate emissions are controlled by bagfilters. Each bagfilter appeared to have regular maintenance conducted. The facility inspects the bagfilters monthly and performs internal inspections. B. 15A NCAC 2D .0521— CONTROL OF VISIBLE EMISSIONS — Visible emissions shall not exceed 20% opacity. APPEARED IN COMPLIANCE: We observed a truck being loaded, and noted brief, light fugitive emissions. No other sources were operating, and no silo filling was observed. There do not appear to be any issues regarding visible emissions at the facilities emission points. The facility has not received any complaints, and no complaints have been received by DAQ. C. 15A NCAC 2D .0535 — NOTIFICATION REQUIREMENT — Notify DAQ in the event of excess emissions lasting longer than 4 hours resulting from malfunctions, breakdowns, or abnormal conditions. Part IV: • Sections A & 8: Revised provisions require stormwater discharges be sampled twice per year (instead of annually), and sampling results shall be compared to new benchmark values. (The previous cut-off concentrations have been removed). • Sections A & B: Revised provisions require the permittee to execute Tier One, Two & Three response actions: Tier One requirements for a first benchmark exceedence, and/or Tier Two response for a second consecutive benchmark exceedence, if applicable. Tier Two requires that the permittee institute monthly monitoring (reported to the appropriate Regional Offices) instead of twice -per -year monitoring. Monthly monitoring shall be done until three consecutive monitoring events show no benchmark exceedences. • Sections A & B. This permit now contains a provision that four exceedences of any benchmark will trigger increased DWQ involvement in the permittee's stormwater management and control actions (Tier Three). DWO may direct the permittee to apply for an individual permit, institute specific monitoring, or may direct the implementation or installation of specific stormwater control measures. • Sections A & B: Failure to perform analytical stormwater monitoring requires that the permittee begin a monthly sampling scheme for both industrial stormwater discharges and/or stormwater discharges from VMA areas (where applicable). After 6 months of monthly sampling, the permittee may return to semi- annual sampling, unless otherwise specified by DWQ. • Sections A & B: A lower TSS benchmark of 50 mg/I for HOW, ORW, PNA and Tr Waters was added to this permit for these more sensitive waters. • Section B: The monitoring parameter Oil & Grease has been replaced by the parameter Total Petroleum Hydrocarbons. • Sections A, B & C: Inability to sample due to adverse weather must be reported in the SPPP. • Sections A, B & C: The terminology "Representative Storm Event" has been replaced by "Measurable Storm Event" in response to comments from industry groups. A measurable storm event is a storm event that results in an actual discharge from the permitted site outfall. In your letter dated August 18, 2004, you stated that all of the stormwater from your site exits as sheet flow and that you have no point -source discharges. In accordance with your request, your Certificate of Coverage (NCG140034) under the NCG140000 permit is rescinded, effective immediately. This rescission is based upon your statement that stormwater from your site exits as sheet flow and confirmation from the Fayetteville Regional Office. A new stormwater permit will be necessaryif the nature of your site changes such that stormwater leaves in a discrete discharge. Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to waters of the State without a valid NPDES permit will subject the responsible party to a civil penalty of up to $25,000 per day. If the facility is in the process of being sold, you will be performing a public service if you would inform the new or prospective owners of their potential need for NPDES permit coverage.
