Please print a copy of the cover letter and two copies of the SOO Monitoring Report, and sign and date inside the gray boxes of the cover letter. Please fill in the date in the date submitted section at the top of the SOO Monitoring Report. Please also sign and date the certification sections on the second page of both copies of the SOO Monitoring Report (note: you will need to sign both the Solvent Management Plan certification on the top of the second page, and permittee certification the bottom on the second page of the SOO Monitoring Report). You should make copies of the letter and SOO Monitoring Report and file them in the monitoring section of your Storm Water Pollution Prevention Plan (SPPP).
