Dear Permittee:
On 2/14/201 1, the Division of Water Quality received your request to rescind your coverage under
Certificate of Coverage Number NCG050332. In accordance with your request, Certificate of Coverage
Number NCG050332 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to
waters of the State without valid coverage under an NPDES permit will subject the responsible party to a
civil penalty of up to $25,000 per day. It is the intention of DWQ that enforcement proceedings will
occur for persons that have voluntarily relinquished permit coverage when, in fact, continuing permit
coverage was necessary. If, in retrospect, you feel the site still requires permit coverage, you should
notify this office immediately. Furthermore, if in the future you wish to again discharge to the State's
surface waters, you must first apply for and receive a new NPDES permit.
If the facility is in the process of being sold, you will be performing a public service if you would inform
the new or prospective owners of their potential need for NPDES permit coverage.

Dear Mr. Miller:
On September 25, 2008, I, Michael Lawyer from the Fayetteville Regional Off ice of the Division
of Water Quality; conducted a site inspection at the Alpha Plastics Division facility located at 106 Marks
Creek Lane in Hamlet, Richmond County, North Carolina. A copy of the Compliance Inspection Report
is enclosed for your review. Mr. Paul Knecht, Technical Manager, and Mr. Paul Kinsey, Plant
Superintendent, were also present during the inspection and their time and assistance is greatly
appreciated. Stormwater from this facility drains to an unnamed tributary to Marks Creek, a Class C
stream located in the Yadkin -Pee Dee River Basin. The site visit and file review revealed that the subject
facility is covered by NPDES Stormwater General Permit-NCG050332.
Accordingly, the following observations were noted during the Division of Water Quality
inspection:

Inspection Summary:
Inspection conducted with Mr. Lee Miller, Plant Manager, Mr. Paul Knecht, Technical Manager, and Mr. Paul Kinsey, Plan]
Superintendent. An overview of facility operations and processes was given followed by a review of the facility's
Stormwater Pollution Prevention Plan (SPPP) and Qualitative Monitoring records. The SPPP is well -detailed and contains
all permit -required components. Monitoring records were also complete and personnel are following the proper monitoring
schedule. After the documents review, a tour was conducted of the manufacturing process as well as the grounds of the
facility including the outfall, which is comprised of a perimeter ditch along the eastern and southern boundaries of the
property that comes together at one point at the southeast corner. As part of the company's proactive approach to
eliminate stormwater contamination, a stainless steel screen/skimmer structure has been installed at the outfall to aid in
the capture of fugitive resin pellets and other solid debris. Facility personnel are well versed in the requirements of the
general stormwater permit and at the time of inspection were in full compliance.

In January 2006, we informed you that Georgia-Pacific Corporation (Georgia-Pacific)
had become a privately held corporation and a wholly -owned subsidiary of Georgia-Pacific
Holdings, LLC, a Delaware limited liability company. Georgia-Pacific Corporation continued to

LP, effective on January 01, 2007, and, once again, it will continue being owned by Georgia-
Pacific. Any permits associated with the facility will be under this new entity from that date
forward. A list of its
issued to the facility is enclosed as Attachment A.
Please let us know what steps, if any beyond this notification, we need to take to effect a
change to our permits. We will assume that this notice is sufficient for your purposes, unless we
hear from you otherwise. Again, it should be emphasized that the facility, under Georgia-Pacific
Consumer Products LP, will continue to be owned within the Georgia-Pacific family of
companies.

In accordance with your application for discharge permit received on October 7, 2004, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES
general permit. This permit is issued pursuant to the requirements of North Carolina General Statute
143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental
Protection agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act, or any other Federal or Local governmental permit that may be required.
