RE: Stormwater Analytical Monitoring Results for Thermo Fisher Scientific, LLC
I have attached the analytical monitoring results from stormwater sampling that was collected on
July 23, 2019 at Thermo Fisher Scientific, LLC's four outfalls.
Per our General Storm Water Permit No. NCG03000 requirements of monthly monitoring for
applicable parameters during a storm event, I have sampled each outfall for copper, zinc, pH,
lead, TSS, and Non -Polar Oil/Grease/TPH. Results for zinc at three of the four outfalls and
copper at one outfall were above the benchmark values, as noted in Table 3 of the general storm
water permit. All other results were below the benchmark values. We have implemented a Tier
Three response for the site and Thermo Fisher Scientific, LLC has notified the Asheville
Regional Office. Thermo Fisher Scientific, LLC is currently working with an environmental
consultant to help achieve compliance with the benchmark values set forth in the facility's storm
water permit. I have attached the completed Monitoring Report containing the monitoring results
from the four outfalls.

Monthly sampling (instead of semi-annual) must begin with the second consecutive benchmark exceedance for the same parameter at the same outfall.
For sampling periods with no discharge at any single outfall, you must still submit this discharge monitoring report with a checkmark here.
The total precipitation must be recorded using data from'an on -site rain gauge. Unattended sites may be eligible for a waiver of the rain gauge requirement.
See General Permit, Section B, Table 1 to identify the especially sensitive receiving water classifications where the more protective benchmark applies.
Total Toxic Organics sampling is applicable only for those facilities which perform metal finishing operations, manufacture semiconductors, manufacture
electronic crystals, or manufacture cathode ray tubes: For purposes of this permit the definition of Total Toxic Organics is that definition contained in the EPA
Effluent Guidelines for the facility subject to the requirement to sample (for metal finishing use the definition as found in 40 CFR 433.11; for semiconductor
manufacture use the definition as found in 40 CFR 469.12; for electronic crystal manufacture use the definition as found in 40 CFR 469.22; and for cathode ray
tube manufacture use the definition found in 40 CFR 469.31).

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TTO
monitoring may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the
General Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO),
I certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the solvent
management plan included in the Stormwater Pollution Preven ion IaIan�."

The total precipitation must be recorded using data from an on -site rain gauge. Unattended sites may be eligible for a waiver of the rain gauge requirement.
See General Permit, Section B, Table 1 to identify the especially sensitive receiving water classifications where the more protective benchmark applies.
Total Toxic Organics sampling is applicable only for those facilities which perform metal finishing operations, manufacture semiconductors, manufacture
electronic crystals, or manufacture cathode ray tubes. For purposes of this permit the definition of Total Toxic Organics is that definition contained in the EPA
Effluent Guidelines for the facility subject to the requirement to sample (for metal finishing use the definition as found in 40 CFR 433.11; for semiconductor
manufacture use the definition as found in 40 CFR 469.12; for electronic crystal manufacture use the definition as found in 40 CFR 469.22; and for cathode ray
tube manufacture use the definition found in 40 CFR 469.31).

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TTO
monitoring may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the
General Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO),
I certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the solvent
management plan included in the Stormwater Pollution Prevent Plan."

Instructions for completing Chain of Custody (COC)
1. Section A and B: Complete all Client information at top of shed company name, address, phone; fax_ contact (the person to contact if thew arc
questions, and who will receive the final report.), c-mail address (if available), PO#, Project Name and/or Project Number as you would like to see it
appear on the report.
2. Section C: Invoice Information: Billing information is included in this section. "flux information should include the name and address of thv person
receiving the invoice.
3. Quote Reference should be completed if quotation was provided by pace Anahtical. The Project Manager, and profile No. will be completed by
pace Anahtical Services.
4. Site Location: A separate COC must be tilted out for each day of sample collection. Record the two letter postal code for the US state in which the
samples wen collected.
5. Re2ulatory Agency: List the prouram that is guiding the work to ensure proper regulations are followed.
6. Section D: Complete a Sample Description in the "SAMPLE ID' section as you would like it to appear on the laboratory report. 'file following
information should also be included: the sample matrix, sample type (G (grab) or C (composite). When collecting a composite, the start time and end
time should be documented in the respective boxes. The collection time for a grab (G) sample should be entered in the boxes marked 'Composite
Eud/Grab'), Sample temp at collection (if required by state), the total number of containers, and preservative used.
7. Mark if the sample was filtered in the field by marking Y or N in `I'iltcred' row by the An lysis requested.
8. Requested Analysis: List the required analvsis and methods on the lines provided and place a check in the column for the samples requiring the
analysis. Additional comments should be referenced in the bottom left hand comer or include attachments for extended lists of parameters.
9. The sampler should print their name in the space provided and sign their name followed by the date of the sampling event at the bottom of the COC in
the spaces designated for `SAMPLER NAME AND SIGNATURE'.
10. When relinquishing custody of the samples to a representative of the laboratory or other organization, indicate the Item Numbers of those samples
being transferred; sign relinquished by, date and time, and include your affiliation.
*Important Note:
Standard Turnaround Time is 2 Weeks/10 business days. Results will be delivered by end of business on the date due unless other arrangements
have been made with },our project manager.
Special Project Requirements such as Low Level Detection Limits or level of QC reported must be included on the chain of custody in the Additional
Comments section.

Monthly sampling (instead of semi-annual) must begin with the second consecutive benchmark exceedance for the same parameter at the same outfan.
For sampling periods with no discharge at any single outfall, you must still submit this discharge monitoring report with a checkmark here.
The total precipitation must be recorded using data from an on -site rain gauge. Unattended sites may be eligible for a waiver of the rain gauge requirement.
See General Permit, Section B, Table 1 to identify the especially sensitive receiving water classifications where the more protective benchmark applies.
Total Toxic Organics sampling is applicable only for those facilities which perform metal finishing operations, manufacture semiconductors, manufacture
electronic crystals, or manufacture cathode ray tubes. For purposes of this permit the definition of Total Toxic Organics is that definition contained in the EPA
Effluent Guidelines for the facility subject to the requirement to sample (for metal finishing use the definition as found in 40 CFR 433.11; for semiconductor
manufacture use the definition as found in 40 CFR 469.12; for electronic crystal manufacture use the definition as found in 40 CFR 469.22; and for cathode ray
tube manufacture use the definition found in 40 CFR 469.31).

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TTO
monitoring may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the
General Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO),

RE: Stormwater Analvtical Monitoring Results for Thermo Fisher Scientific, LLC
I have attached the analytical monitoring results from stormwater sampling that was collected on
June 9, 2019 at Thermo Fisher Scientific, LLC's three outfalls.
Per our General Storm Water Permit No. NCG03000 requirements of monthly monitoring for
applicable parameters during a storm event, I have sampled each outfall for copper, zinc, pH,
lead, TSS, and Non -Polar Oil/Grease/TPH. All results were within the benchmark values as
noted in Table 3 of the general storm water permit. We have implemented a Tier Three response
for the site and Thermo Fisher Scientific LLC has notified the Asheville Regional Office. Thermo
Fisher Scientific LLC is currently working with an environmental consultant to help achieve
compliance with the bench mark values set forth in the facility's storm water permit. I have
attached the completed Monitoring Report containing the monitoring results from the three
outfalls.
