Dear Sir or Madam:
Per our NPDES Stormwater Permit NCG030000, Industrial Connections & Solutions LLC collected the
required benchmark samples from our facility outfalls on June 7, 2019. The following documents are en-
closed:

In accordance with our NPDES Stormwater Permit, Industrial Connections & Solutions LLC is notifying
the Division of Energy, Mineral, and Land Resources (DEMLR) of benchmark value exceedance for Out -
falls SDO-5 & SDO-6 for copper. Outfall SDO-6 for zinc and Outfall S130-6 for pH. We will continue to
conduct monthly sampling while looking for solutions to these benchmark exceedances.

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TTO monitoring
may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General
Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics
(TTO), I certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the
solvent management plan included in the Stormwater Pollution Prevention Plan."

Dear Ms. Stennis
As authorized by acceptance of our proposal dated September 2016 and your requests to continue
sampling, Blue Ridge Geological Services, Inc (Blue Ridge) personnel performed additional
environmental services at the subject site. The work was performed in general accordance with the
Stormwater Pollution Prevention Plan (SWPPP) prepared for the subject site as well as the requirements of
NCDEQ NPDES General Permit. On June 7, 2019 Blue Ridge personnel performed a stormwater
inspection and sampling event at the subject site during a measureable rainfall event. Rainfall data was
obtained from a nearby weather station. According to the website, the area in the vicinity of the subject site
received approximately 0.41 inches of rain during the storm event.
Blue Ridge personnel observed the stormwater discharge at outfalls SDO-1 through SDO-7 and completed
stormwater discharge outfall (SDO) qualitative monitoring report forms for each of the outfalls. The SDO
forms are attached. The pH of the samples was measured in the field using an Oakton pH meter and
recorded on the SDO forms. No environmental issues were observed at the outftils during this inspection.
Field personnel collected samples from each of the stormwater outfalls, placed the samples into labeled
containers, completed a chain of custody (COC) form, and submitted the samples and COC form to Meritech
personnel.
This inspection report and the SDO forms should be placed in the SWPP Plan or a file on site and they do not
need to be submitted to the NCDEQ. We will attempt to perform the next stormwater inspection/sampling
event at the site before July 31, 2019. We appreciate the opportunity to continue to assist you on this
