On July 5, 2013, the Division of Energy, Mineral and Land Resources received your request to rescind
your coverage under Certificate of Coverage Number NCG020553. In accordance with your request,
Certificate of Coverage Number NCG020553 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to
waters of the State without valid coverage under an NPDES permit is against federal and state laws and
could result in fines. If something changes and your facility would again require stormwater or
wastewater discharge permit coverage, you should notify this office immediately. We will be happy to
assist you in assuring the proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would inform
the new or prospective owners of their potential need for NPDES permit coverage.

This rescission request is for a mine currently covered under COC NCG020553. The "Individual Permit" number that was
erroneously entered on the rescission form is the facility's mining permit number issued by LQ. This mine has been
undergoing reclamation. I spoke with the mine owner (Mr. Tew) last week and he relayed that the site has been fully
reclaimed and the bond with LQ released. I would like to verify this with LQ staff before saying it is OK to rescind.

requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as
subsequently amended.)

Your coverage under the General Permit is transferable only through the specific action of DWQ.
