On 7/22/2009, the Division of Water Quality received your request to rescind you coverage under Certificate of Coverage
Number NCG020269. In accordance with your request, Certificate of Coverage Number NCG020269 is rescinded
effective inunediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to waters of the State
without valid coverage under an NPDES permit will subject the responsible party to a civil penalty of up to $25,000 per
day. It is the intention of DWQ that enforcement proceedings will occur for persons that have voluntarily relinquished
permit coverage when, in fact, continuing pennit-coverage was necessary. If, in retrospect, you feel the site still requires
permit coverage, you should notify this office immediately. Furthermore, if in the future you wish to again discharge to
the State's surface waters, you must first apply for and receive a new NPDES permit.
If the facility is in the process of being sold, you will be performing a public service if you would inform the new or
prospective owners of their potential nee for NPDES permit coverage.

Dear Mr. Kirkpatrick:
On July 23, 1996 the Division of Environmental Management received your Notices of
Intent requesting coverage under general stormwater permit NCG020000. Staff review of your
application materials showed that there is no point source discharge from your Hoffman Sand Pit
facility because all of the stormwater drains into the sand pit where it is absorbed. This was
confirmed with Mr. Joe Glass in the Fayetteville regional office. Because there is no stormwater
point discharge from this facility it is exempt from the stormwater permit requirements. The
Division is therefore returning your application.
Review of the application materials for your Hall-Rackley Pit showed that a certificate of
coverage number for a stormwater permit was given. Opening the file of the existing certificate of
coverage confirmed that this site located on S.R. 1001 in Harnett county is already covered by
NCG02000, with certificate of coverage number NCG020269. A copy of your existing certificate
of coverage is enclosed along with your most recent application materials for your records.
Your application fees for these two facilities ($800.00) will be refunded and sent under a
separate cover.

Our files indicate that the subject permit issued on 8/22/89 expires on
8/30/94. We have not received a request for renewal from you as of this date.
A renewal request- shall consist of a letter requesting renewal and completion of
the enclosed application, submitted in triplicate. A processing fee must be submitted
with the request. Please find attached a copy of the 15 NCAC 211 .0205(c) regulations.
The processing fee for your facility is based on the design or permitted flow,
whichever is appropriate, listed in the first four categories of facilities. Sludge
renewals are based on total acres of land permitted. If the facility covered by this
permit contains some type of treatment works, a narrative descripton of the sludge
management plan that is in effect at the facility must be submitted with the
application for renewal.
The Environmental Management Commission adopted revised rules on October 1, 1990
(attached), requiring the payment of an annual fee for most permitted facilities. You
will be billed separately for that fee (if applicable), after your permit is approved.
Please be advised that this permit must not be allowed to expire. If the renewal
request is not received within 180 days prior to the permit's expiration date as
required by 15 NCAC 2H .0211, you will be assessed an automatic civil penalty. This
civil penalty by North Carolina General Statute may be as much as $10,000 per day. If
a permit renewal request is not received 180 days before permit expiration, a cavil
penalty of at least $250 will. be assessed. Larger penalties may be assessed depending

on how late the request is made. In addition, any permit renewal request received
after the permit's expiration date will be considered as a new application and will be
required to pay the higher permit application fee.

PERMISSION IS HEREBY GRANTED TO
G.S. Materials, Inc.

FOR THE
construction and operation of a 270,000 GPD recycle facility consisting of. an 1,800
GPM pump, 1.,630 linear feet of 8-inch force main, a gravity drainage ditch, a.
settling pond and clear wager pond with combined capacity of approximately 10 MG with
no discharge to the surface waters of the State, pursuant to .the application
received July 3, 1.989, and in conformity with the project plan, specifications, and
other supporting data subsequently filed and approved by the Department of Natural
Resources and Community Development and considered a part of this permit.
This permit shall. be effective from the date of issuance until August 30, 1994,
and shall. be subject to the following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at 8.11. times.
4. This permit is not transferable.
5. Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professionalengineer
certifying that the permitted facility has been installed in accordance with the
requirements of this permit and the approved plans and specifications. Mail the
certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, N.C.
27611-7687.

6. A set of approved plans and specifications for the subject project must be
retained by the applicant for the life of the project.
7. Tn the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of: additional. or replacement wastewater treatment or disposal. facilities.
8. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will be established and an acceptable
sampling reporting schedule shall be followed.
9. The solids generated from these treatment facilities must be disposed of in
accordance with General Statute 143-215.1 and in a. manner approvable by the North
Carolina Division of Environmental Management.
10. Diversion or bypassing of the untreated wastewater from the recycle system is
prohibited.
11. Freeboard in the settling pond and clear water pond shall not be less than two
(2) feet at any time.
12. Adequate facilities shall be provided to prevent surface runoff- from carrying any
disposed or stored material into any surface waters.
13. The facilities shall be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of
this facility.

14. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, and if warranted, will.
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
15. The annual administering and compliance fee must be paid by the Permittee within
thirty (30) days after being billed by the Division. Failure to pay the fee
accordingly, may cause the Division. Failure to pay the fee accordingly, may
cause the Division to initiate action to revoke this permit, as specified by 15
NCAC 211 .0205(c).

PART I. INSPECTION OF PROPOSED WASH WATER RECYCLE SYSTEM SITE
1. Place Visited: GSM, Inc., Hall-Rackley Mine

5. Directions to Site: The Hall-Rackley Mine is located on the east side
of SR 1001, approximately 1 mile off NC 24-27 in Harnett County.
6. Size: The original development of the Hall-Rackley Mine included
approximately 67 acres. This proposed addition involves another
85 acres located immediately adjacent to the original site.
7. Topography: Rolling.
8. Location of nearest dwelling: Approximately 1000 feet from the plant
site.
9. Receiving stream: N/A
PART II - DESCRIPTION OF TREATMENT WORKS
1. Type of waste: Wastewater at this facility will consist of water used
in a sand washing operation, mingled with silts and sands removed during
that process.
2. Size of facility: This facility is expected to produce 300-375 ton/hr
of washed sand. This system is designed to provide 1800 gpm of settled
water back to the washing -screening equipment.
3. Type of treatment: GSM proposes to use two settling basins in series to
provide physical treatment for this wash water. The primary basin,
approximately 8' deep, will have a surface area of + 1.75 acre. The
secondary basin will contain approximately 3.5 acres at a depth of.
10-12 feet. Mr. Harmon indicates an estimated total capacity of
10,000,000 gallons. Using 1800 gpm and assuming a 24-hr. operation,
this should provide more than adequate retention time (approximately
96 hours).

No outlet to surface waters is to be constructed.
Notes indicate that all runoff from the site is to be collected in the
wash water settling basins. Unless it is necessary to collect this
runoff for use as make-up water, it should be excluded from these two
basins. Due to the extremely sandy nature of this site, surface runoff
is expected to be minimal. If necessary, additional sediment storage
area can be found in sections of the site that have already been mined.
PART III - EVALUATION AND RECOMMENDATION
It is the recommendation of this office that Permit No. 11195, issued to
Mr. R. G. Kirkpatrick for GSM, Inc., be amended to include the above described
wastewater treatment facilities. This recommendation is based on the deter-
mination by this office that the site appears suitable for the size and type
system proposed.

The Permits and Engineering Unit has completed a preliminary engineering review of the
subject application.
1. Your permit (#11195) is for the recycle system only, we do not permit the actual
mine acerage. Do I understand correctley that you are aboandoning your currently
permitted recycle system.
2. Is the 10 MG capacity after or before 2 feet of freeboard is provided for ?
3. Is the total average daily flow still 150,000 gallons ?
4. What is done with the accumulated solids that result from this operation ?
The following items must be addressed before we can complete our review. Please note that
these items must be received by Sept. 17, 1989 in order to prevent your project form being
returned as incomplete:

GENERAL SPECIFICATIONS
PUMP
The pump shall be an Allis -Chambers Series 8000 8 x 8 x 17
- Curve A7498-2 1800 GPM at 220 TDH. The pump shall be
driven by a 150 HP - 1800 RPM-TEFC-460/3/60 Motor. (See
attached pump and motor data.)
The force main shall be an 8" diameter. Material shall be
NYPAO-150 psi rated with manufactured joints. Piping shall
be installed in accordance with manufacturers
recommendations.
GATE VALVES AND CHECK VALVES
All check valves shall be manufactured by Mueller or equal
and be of sue as shown on plans.
ELECTRICAL
All electrical work shall be performed by a licensed
electrical contractor with -a letter of certification to
owner that the electrical work has been installed in
accordance with State and all local electrical codes.
EROSION CONTROL
All Erosion Control methods shall be installed in
accordance with the North Carolina Sedimentation Control
Commission and in accordance with approved erosion control
plan.
PORTABLE SAND WASHING UNIT
300-375 Toms per hour, nine station dial split unit at
manufactured by Eagle Iron Works, De Moines. Iowa.
