4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
Since this is an after the fact permit application the existing conditions are a new four span 332' long by 33' wide bridge. The previous conditions were a deteriorating six span 330' long
by 28' wide bridge. Land use has not changed from mostly forested and bridge is on paved secondary road.

4h. Explain the purpose of the proposed project:*
NCDOT Bridge Management Unit indicated Bridge No. 57 had a low sufficiency rating and was approaching the end of its useful life. Therefore the purpose of the work was to replace
the deteriorating bridge for the safety of the travelling public.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
NCDOT has replaced Bridge No. 57 on SR 1248 (Southfork Road) over South Fork Catawba River in Lincoln County south of Lincolnton. The previous 330' long by 28' wide six span
deteriorating bridge was replaced with a 332' long by 33' wide four span bridge. The new bridge has no direct discharge into the water. As recommended by DWR the new bridge will
have Class II bank stabilization added to both banks under bridge.
All Best Management Practices will be utilized during the course of the project. The project will be constructed using typical heavy equipment (trackhoe, backhoe, etc).

The bridge was replaced on the wasting alignment. An offsite detour was used to maintain traffic during construction. Bents were located as close to
bank as possible. Deck drains only outside Top of Bank. Drainage was designed to maintain previous drainage patterns and have as little
environmental and surface water impacts as possible. Class II rip rap was used to stabilize the banks under the new bridge.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
Minimization of impacts by installing appropriate erosion control measures. Minimal impact to stream during old bridge removal.
