In Amy's absence can you please help ensure this makes its way to the right person? Note that we had previously
provided the revised table of buffer impacts from the PCN to Amy to obtain a modification to the 401 WQ Certification
previously issued. Amy advised me via email that we also needed to re-submit the signature page (attached). We also
received a letter that I believe you may have prepared indicating the same.

Project is to be located in DOT right-of-way, 5 feet off the edge of pavement. Impacting federally protected species is
highly unlikely and difficult to quantify since the habitats of the species in concern are not delineated.

6a. Will this project occur in or near an area designated as essential fish habitat? ? Yes ® No
6b. What data sources did you use to determine whether your site would impact Essential Fish Habitat?
Research was conducted using the USFWS and various other sources. No substantial evidence could be found to
assume the unnamed tributaries of East Mingo Creek and the unnamed tributaries of Hannah Creek could be considered
Essential Fish Habitat.
7. Historic or Prehistoric Cultural Resources (Corps Requirement)
7a. Will this project occur in or near an area that the state, federal or tribal
governments have designated as having historic or cultural preservation ? Yes ® No
status (e.g., National Historic Trust designation or properties significant in
North Carolina history and archaeology)?
