Division of Water Resources' Central and Regional staff has reviewed the application package
received November 12, 2019. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on February
17, 2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through P, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

A. Cover Letter and Project Narrative:
1. Revise the project narrative to clearly explain which treatment and irrigation units are changing in
the proposed design, and which units will be remaining as previously designed. Any units that are
not being modified (i.e. not modifying the unit or the location) do not need to be reviewed again
during this modification.

1. The applicant listed in Section I Item 3 is not the current deeded property owner for this site. The
provided deed (Book: 2060, Page: 547-549) shows Western Wake Investments, Inc. sold the
subject property to Donald C. Jackson of the Donald C. Jackson Revocable Trust, Katherine M.
Jackson Trustee of the Katherine M. Jackson Revocable Trust, and Emma G Jackson and spouse
Ismael A. Hernandez Cruz on August 13, 2019.
In accordance with 15A NCAC 02T .0604(e), Property Ownership Documentation shall be
provided to the Division consisting of -
legal documentation of ownership, such as a contract, deed, or article of incorporation;
ii. an agreement of an intent to purchase the property that is written, notarized, and signed by
both parties, accompanied by a plat or survey map; or
iii. an agreement to lease the property that is written, notarized, and signed by both parties,
indicating the intended use of the property, accompanied by a plat or survey map. Lease
agreements shall adhere to the requirements of 15A NCAC 02L .0107.
Either change the Applicant to the deeded property owner or provide item ii or iii listed
above.
2. It is checked that setback waivers are proposed for this facility in Section V Item 20. The Division
did not receive a copy of this setback waiver. Provide a copy of the setback waiver or mark "no"
in the application.
3. In Section VI Item 2, if any of the listed units are not being modified from when the system was
previously permitted, please write "previously permitted" in the specification reference column or
leave these units out of the application completely.
4. It is unclear if the irrigation system is being modified or not. If the irrigation field is not being
modified, leave this section of the application blank.

D. Existing Permit
1. The existing permit provided and listed in Application Section IV.1. is not the most recent permit.
The most recent permit was issued November 10, 2016 and can be found in our online records
portal. No response is required for this comment.
E. Property Ownership Documentation:
1. A deed was provided for the subject property; however, the deeded property owner is not listed as
the Applicant. See item C.1.
F. Soil Evaluation:
1. A soil evaluation is required for application packages that include new irrigation sites, relocating
irrigation sites, or increasing loading rates on irrigation sites. If this modification does not include
modified irrigation sites, do not submit a soils evaluation. If this modification does include
modified irrigation sites, a statement shall be included indicating that the site has not changed since
the original investigation because the evaluation was prepared more than one year prior.
G. Engineering Plans:
1. Make it clear in the Engineering Plans what is being modified at the site. Create a plan sheet
showing the entire site and label anything that is not being modified "as previously permitted." For
example, if the 1,500 gallon baffled septic tank is being installed in the same location as the
currently permitted 1,500 gallon baffled septic tank, then label it "as previously permitted" so it is
clear this unit is not being modified and therefore does not need to be approved again.
2. Provide detail drawings of all proposed pumps.
3. Provide a hydraulic profile from the treatment plant headworks to the highest irrigation point.
H. Specifications:
1. Only provide specifications for units that are being modified in this application. Units that are not
being modified shall be constructed with the previously -approved plans and specifications.
