submittal package, inclusion of these materials under my signature and seal signifies that I have reviewed this material and
have judged it to be consistent with the proposed design.
NOTE — In accordance with General Statutes 143-215.6A and 143-215.613, any person who knowingly makes any false
statement, representation, or certification in any application package shall be guilty of a Class 2 misdemeanor, which may
include a fine not to exceed $10,000, as well as civil penalties up to $25,000 per violation.

the subject project.
This project includes a new sewer extension of approximately of 3721f of 8" SDR 35 PVC
sanitary sewer line. The sewer line is being extended to serve 12 single-family lots.
Please find enclosed:
1. This cover letter with the project description in the paragraph above;
2. A check in the amount of $480.00 for the distribution system fee;
3. The original and one copy of the completed "Fast Track Sewer System Extension
Application" (FTA 04-16);
4. A completed "Flow Tracking for Sewer Extension Applications" (FTSE 04-16);
5. One 8 % x 11 color copy of the USGS topographic map of this area;
6. One copy of the street level map showing the general project area.
