I further certify that the Applicant or any parent, subsidiary, or other affiliate of the Applicant has not been convicted of an environmental crime, has not abandoned a wastewater facility without proper closure, does not have an outstanding civil penalty where all appeals have been exhausted or abandoned, are compliant with any active compliance schedule, and does not have any overdue annual fees. NOTE — In accordance with General Statutes 143-215.6A and 143-215.6B, any person who knowingly makes any false statement, representation, or certification in any application package shall be guilty of a Class 2 misdemeanor, which may include a fine not to exceed $10,000 as well as civil penalties up to $25,000 per violation. This facility is located at address 2607 Old Highway 421, south of the intersection of US-421 and Old Highway 421 (exit 180). Combined, the mill and hatchery employ 33 employees for first shift. Both facilities average approximately 5,000 gallons daily discharge to the treatment lagoon. Based on this rate, it is estimated that amount of industrial contribution is 84% and the domestic (based on the number of employees at 25 gallons per day) is 16%. Other records indicate that the percentage is likely to be 95% industrial and 5% domestic, so the numbers provided are a worse -case calculation. The hatchery is tasked with hatching and providing baby chicks to Pilgrim's growers in the area. Eggs are incubated until hatched and delivered to farms on a weekly basis. Regular housekeeping through washdown events generate wastewater. The majority of the water sent for treatment is generated in this way. Broken or unhatched eggs are placed in a roll -off box or truck and allowed to drain. The drained albumin is sent to the package plant for processing. Shells are sent to the landfill via a rolloff box or truck. The feed mill is tasked with producing feed and delivering it to Pilgrim's growers in the area. Grain is milled and blended and delivered to growers on an as needed basis with trucks delivering daily. Regular housekeeping through washdown events and boiler blow -down generate wastewater. Wastewater for this is limited and occurs at the end of shift to allow drying. The majority of the feed mill water sent for treatment is generated in this way. Effluent Process Narrative Process Narrative Wastewater is treated through a precast concrete package plant. The wastewater generated collects in one of two concrete tank groups. Each of these groups has a total water holding volume of between 1,000 gallons and 2,000 gallons. Effluent enters each group via gravity. Effluent is pumps with a grinder pump from the two tanks to an equalization tank because flow is not consistent. From the equalization tank, effluent is sent to a Pollution Control Model SS-20-SP 10,000 gallon per day package plant. From the package plant effluent enters the aerobic treatment lagoon. This is a lagoon with roughly 200,00 gallons of storage. Effluent entering the lagoon will have large solid particles retained in the collection tank or package plant prior to entering the treatment lagoon. Effluent is then transferred via a flashboard riser pipe to the storage lagoon. This lagoon has roughly 400,000 gallons of storage. Treated effluent from the storage lagoon is pumped to the spray field when conditions are acceptable to spray. The spray field is composed on 5 lines with 11 heads on each line. The total spray field is 6.27 acres. Irrigation is limited to an hourly rate of 0.3 inches per hour and a maximum yearly application rate of 35.88 acre inches per year. Annual irrigation amounts average 14 acre inches for this facility.
