A new eNOI will need to be submitted with the following corrections made: Part Ala) Project Name does not match the Project Name provided on the E&SC Letter. Part A3)Address does not match E&SC Approval Letter. Only the Street Address should be provided here, or street name if no address number has been assigned yet. Part 7/8) Coordinates are located on Canal Street, not at the address provided in Part A3. Part Al2)Acres to be disturbed (4.00 acres) do not match the acres approved in the E&SC Approval Letter(13.85 acres). Either the acres should match or a new E&SC Approval Letter should be obtained from the DEQ Wilmington Regional Office. Part A15b)The index number for Southwest Creek is 19-17-(0.5). Part 136)An email was submitted in the street address line. Part D1) Date does not match E&SC Approval Letter. Part D2) Project ID (10-32-00) does not match E&SC Approval Letter(ONSLO-2020-007). Part D3) E&SC Approval Letter was approved by a State DEQ Office. Part D4) State DEQ Office is the Wilmington Regional Office. Part D5) Based on the number of submittals that do not match the E&SC Approval Letter, it is possible the wrong E&SC Approval Letter was submitted in this application. Please verify the correct E&SC Approval Letter is submitted. Part D6)An NOI Certification Form was not submitted. The NOI Certification Form is hyperlinked in Part D of the eNOI. To access, click the blue letters that say: "Notice of Intent(NOI) Certification Form." Part E)The person who signs the NOI Certification Form and signs the Certification in Section E of the eNOI should be the same person as listed in Part B, or an authorized responsible individual within the same organization.
