This Permit shall be effective from the date of issuance until rescinded

ns and limitations:
1. This permit shall become void unless the facilities are constructed in accordance
with the approved plans, Specification* and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described
In the application and other supporting data.
3. The facilities shall be properly maintained and operated at all tines.
4. This permit shall become void unless the agreement between the Mt. Sinai Holy Church
Development Corporation and the City of Raeford for collection and final treatment of the
Corporation's domestic wastewater, is in full force and effect.
S. The sewage and wastewater collected by tbis system shall be adequately treated in
the Raeford Wastewater Treatmeat Pleat prior to being discharged into Reakfish Creek.
