Dear Mr. Smith:
Enclosed you will find the reviews for the 2019 Annual Report (AR) and a Compliance Evaluation
Inspection (CEI) for the above -stated permit. The AR inspection revealed an issue that needs to be
addressed with regards to several Chains of Custodies presented in the AR.
All samples should be preserved on ice. The lab receiving temperature for the samples should
be recording a sample receiving temperature below 6° Celsius. If necessary, the contract
laboratory should be able to provide coolers to be able to ship the samples that require outside
laboratory analysis.
The Annual Report was found to be in compliance with the permit.
The CEI was performed as a part of the permit renewal process. The facility and residual reduction
processes appear to be in compliance with the Class "A" Distribution permit for this facility.
