Inspection Summary:
On 6/27/2018, Jason Gill and Melissa Joyner with DEMLR met with Scott Byrd, Plant Manager, at Oldcastle Stone
Products/Bonsal Mining offices to conduct a follow-up compliance inspection for the purpose of reviewing the components of
the facility's Stormwater Pollution Prevention Plan (SPPP) that was unable to be located during DEMLR's inital compliance
inspection on 4/11/2018. The facility had produced a SPPP, but it was identified as being specific to another mine operated
by the same owner in close proximity to Richardson Mine. This SPPP did have some information specific to Richardson
Mine, such as a site map, qualitative monitoring forms, and laboratory analysis results for its quantitative monitoring
requirement. The Richardson Mine site map was missing drainage flow direction arrows and a label for the one outfall at the
mine. Mr. Byrd indicated to the inspectors that he could have the map updated to include the missing information within a
couple of weeks. Even though there was no SPPP on site, the facility was conducting monthly qualitative monitoring of the
outfall and required analytical monitoring in accordance with permit specifications. Mr. Byrd said that he was going to
reduce the monthly qualitative monitoring to semi-annually in accordance with permit specifications. The facility had not
been completing and submitting Discharge Monitoring Reports for Richardson Mine, but the inspectors discussed this
requirement with Mr. Byrd. It was brought to Mr. Byrd's attention that a separate SPPP specific to Richardson Mine should
be maintained on site.

On April 11, 2018, you and your team conducted an onsite inspection and the following issues
were noted:
a. No Storm Water Pollution Prevention Plan (SPPP) developed.
b. Qualitative Monitoring had not been conducted or recorded in accordance with permit
requirement.
c. Analytical Monitoring had not been conducted or recorded in accordance with permit
requirement.
The reason for this is that the Storm Water Pollution Prevention Plan (SPPP) binder / records
had been misplaced during the restructuring of the office area.
We have located the records and the plan of Action now is to maintain these records in the
Safety Representatives office (Mr. Billy Thompson).

On April 11, 2018, Melissa Joyner, Jason Gill and Tim LaBounty from the Fayetteville Regional Office of the
Division of Energy, Mineral and Land Resources (DEMLR), conducted a site inspection for the Oldcastle
Stone Products/Bonsal Mining — Richardson Mine facility located on US Hwy 74 East, in Anson County,
North Carolina. A copy of the Compliance Inspection Report is enclosed for your review. Scott Byrd, Plant
Manager, was also present during the inspection and his time and assistance is greatly appreciated. The site
visit and file review revealed that the subject facility is covered by NPDES Stormwater General Permit
NCG020000 under Certificate of Coverage NCG020259. Permit coverage authorizes the discharge of
stormwater from the facility to receiving waters designated as an unnamed tributary to Smith Creek, a Class
WS-IV waterbody located in the Yadkin -Pee Dee River Basin.
As a result of the site inspection, the following permit conditions violations are noted:
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has not been developed.
2) Qualitative Monitoring
Qualitative monitoring has not been conducted and recorded in accordance with permit requirements.
3) Analytical Monitoring
Analytical monitoring has not been conducted and recorded in accordance with permit requirements.
Other Observations:
Please refer to the enclosed Compliance Inspection Report for additional comments and observations made
during the inspection.
Requested Response:
You are asked to respond to this office, in writing, within 30 calendar days from receipt of this notice. Your
written response should include a reasonable explanation as to why the aforementioned violations have
occurred as well as a Plan of Action to prevent these violations from recurring.

Inspection Summary:
On April 11, 2018, an inspection was conducted at the Richardson Mine by Melissa Joyner, Jason Gill and Tim LaBounty to
determine compliance with the General Permit NCG020000. The Stormwater Pollution Prevention Plan which had been
available for review during the inspection on June 9, 2016 had been misplaced and was not available for review during this
inspection and not provided within 30 days from the date of this inspection. No Qualitative Monitoring has been conducted.
Analytical Monitoring was noted for December 2016 and for March 2018. No benchmark values were exceeded during those
monitoring periods. No monitoring had been performed in the Period One of 2016 or during 2017. Per the requirements of
General Permit NCG020000, both Analytical and Qualitative Monitoring need to occur semi-annually. A Notice of Deficiency
was sent previously to the facility on June 16, 2016 for conducting Analytical and Qualitative Monitoring annually instead of
semi-annually.
Outfall #1, the only stormwater outfall at this mine was visually inspected. It was actively discharging with no issues. Gravel
haul road entrances had not been installed for the mine from public roads and there is a potential for off -site sediment.

I am aware of the deficiency noted in Melissa report and discussed it with her at length. Communication
and or a misunderstanding was the cause of the deficiency. We have been sampling once per year for
some time now. Although the plan calls for semi-annual sampling I failedAo recognize the change. I assure
you we will begin to monitor correctly in the future. Thanks for making us aware of the problem and we
appreciate the help your department provides for us.

On June 9, 2016, Melissa Joyner and Timothy LaBounty from the Fayetteville Regional Office of the Division of Energy,
Mineral and Land Resources, conducted a site inspection at the Oldcastle Stone Products/Bonsal Mining — Richardson
Mine facility located on US Hwy 74 East in Anson County, North Carolina. A copy of the Compliance Inspection Report is
enclosed for your review. The site visit and file review revealed that the subject facility is covered by NPDES Stormwater
General Permit NCG020000, Certificate of Coverage NCG020259. Permit coverage authorizes the discharge of
stormwater from the facility to an unnamed tributary to Smith Creek, a Class WS-IV water located in the Yadkin -Pee Dee
River Basin.
As a result of the site inspection, the following deficiencies are noted:
1) Qualitative Monitoring
Qualitative monitoring has not been conducted and recorded in accordance with permit requirements.
2) Analytical Monitoring
Analytical monitoring has not been conducted and recorded in accordance with permit requirements.

Please refer to the enclosed Compliance Inspection Report for additional comments and observations made during the
inspection.
Requested Response:
You are asked to respond to this office, in writing, within 30 calendar days from receipt of this notice. Your written
response should include a reasonable explanation as to why the aforementioned deficiencies have occurred as well as a
Plan of Action to prevent these deficiencies from recurring.

Inspection Summary:
An inspection was conducted at the Richardson Mine on 6/9/2016 by Melissa Joyner and Tim LaBounty. The Stormwater
Pollution Prevention Plan was reviewed and appeared to contain the required components. The site map needs to be
modified to contain the required information as described in General Permit NCG020000.
Since the permit was renewed on October 1, 2015, Analytical and Qualitative Monitoring records were reviewed for the
previous permit cycle. It was noted that both types of monitoring were being conducted once per year instead of biannually
as required by General Permit NCG020000. No stormwater discharge exceedances were observed of the benchmark
values. The Annual Summary Discharge Monitoring Reports (DMR's) are required to be submitted annually to the Central
Office. Outfall #1, the only outfall at this mine, was inspected and appeared in good condition.

1704 in Anson County, North Carolina. A copy of the Compliance Inspection Report is enclosed for your review. Mr. T. F.
Boone, Superintendent, and Mr. Mitchell Ratliff, Supervisor, were also present during the inspection and their time and
assistance is greatly appreciated. Stormwater from this facility drains to an unnamed tributary to Smith Creek, a Class
WS-IV water located in the Yadkin -Pee Dee River Basin. The site visit and file review revealed that the subject facility is

Inspection Summary:
Due to the effective date of the current NCG020000 general permit being January 1, 2010, this inspection was conducted
to evaluate compliance with the conditions of the previous permit that was effective from January 19, 2005 through
December 31, 2009. Requirements of the previous permit included the development of a Stormwater BMP Plan,
Analytical and Qualitative Monitoring for stormwater only discharges, and Analytical Monitoring for mine dewatering and
process wastewater discharges. Inspection conducted with Mr. Ron Barnhill, General Manager, Mr. Mitchell Ratliff,
Supervisor, and Mr. T. F. Boone, Superintendent. Oldcastle Stone Products owns and operates three mining facilities,
one in Richmond County (Woodmont Mine) and two in Anson County (Federal/Richardson Mine & Lilesville Mine) that are
in close proximity to each other. A Stormwater BMP Plan was developed that encompasses these three mining facilities
and was reviewed at the Oldcastle office located on Hwy 74 East in front of the Lilesville Mine. The BMP Plan appeared to
contain all required components. Analytical Monitoring records for each of the three mines were also reviewed. Based on
laboratory reports, testing results for all parameters showed no exceedances of Cut-off Concentrations that were
applicable under the previous permit. Qualitative Monitoring records were not available at the time of inspection, but
copies were received via fax on February 17, 2010 as requested.
Specific to the Federal/Richardson Mine, activities consist solely of material excavation and transport to the Lilesville Mine
for processing. No mine dewatering or process wastewater discharges occur so monitoring has been performed for
stormwater discharges only.

pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.

Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law rule, standard, ordinance, order, judgment, or
decree.
Please note that in 1998 Senate Bill 1366 established changes to the permit fee structure for DWQ permits effective
January 1, 1999. This change requires that you pay an annual fee to assure continued coverage under this permit.
You will be invoiced for this fee beginning next year. A copy of the current fee schedule is included with this letter.
