This analysis has been performed in accordance with local established policies and procedures using the best available data. This certification applies to those items listed above in Sections I and II plus all attached planning assessment addendums for which I am the responsible party. Please note that this package includes two separate applications. The first is for the lift station and force main on the school property. This infrastructure will be owned and operated by Pender County Schools. The second is for a force main along Hoover Road from the School to US Hwy 17 to connect to the existing 10" force main. Please note that the force main along Hoover Road is 6" and the pumps in the proposed lift station will not provide scour velocity. Instead this line has a stub for a mobile pump to be hooked up to flush the line periodically. This package includes the application fees and two copies of: • Two copies of application form FTA 04-16 for Pender County Schools. • Two copies of application form FTA 04-16 for Pluris Hampstead, LLC • Two USGS maps showing the site. • Two sets of the utility master plan. • Two copies of the FTSE for the project. (This is for the School permit, the force main along Hoover Road is zero flow.) • Two application fees of $480 each. Please let the following serve as a brief narrative for the project: Pender County Schools has an urgent need to connect to the Pluris Hampstead collection system due to issues with their onsite treatment and disposal system. This project proposes using an existing wet well for the proposed lift station. This lift station pumps to a 3" force main. This lift station and 3" force main will be owned and operated by Pender County Schools. The FTSE for this project will go along with this permit. The schools currently have less than 400 students. The lift station is sized for 500 students to leave room for some future growth. The 3" force main will pump to a 6" force main that runs from the School to Hwy 17 along Hoover Road. This line is larger to accommodate future flows along that corridor. Due to the increase in size, line flushing can be accomplished by using a portable pump to flush the line as needed. There is a stub for the pump connection on the end of the line for this purpose. gallantmc@yahoo.com tel 910.448.1046 P.O. Box 4039 Surf City, NC 28445 Michael C.
