NCO024244 I11 121 18/08/13 117 18 1 RI Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) This facility reported loss of power the night of August 11, 2018 and the morning of August 12, 2018 between 11:00 pm and 07:00 am as the result of a thunderstorm. The standby generator did not start up and, as a result, 478,333 gallons of partially treated sewage overflowed a lift station and went into Long Creek. It was noted at time of site visit that generator is a 1972 model generator with manual switch gear. Please be advised, that in accordance with 15A North Carolina Administrative Code (NCAC) 2H .0124, Reliability, "All facilities shall provide adequate measures, which, in the opinion of the Director, will insure continued treatment and disinfection where the interruption of such treatment would render the waters unsafe for their best intended uses. The relibility measures shall include... (2)(a) dual or standby power supply on site" Also in accordance with NPDES Permit NC0024244, Part II, Section C, Item 2. Proper Operation and Maintenance, "The permittee shall at all times provide the operation and maintenance resources necessary to operate the existing facilities at optimum efficiency. The Permittee shall at all times properly operate and maintain all facilities and systems of treatment and control (and related appurtenances) which are installed or used by the Permittee to achieve compliance with the conditions of this permit..."
