However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on April 16, 2020. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Application (15A NCAC 02T .0105): 1. Verify the new facility name. 2. Provide easements for monitoring wells MW-4 and MW-7, which are located on property not owned by the Permittee. B. Site Map: Provide an updated site map in accordance with 15A NCAC 02T .0105(d) that shows the following: 1. Legend, north arrow, and scale 2. Distance measurements verifying all setbacks are being met 3. Stormwater drainage controls 4. Location and ownership of property boundaries within 500 feet of the disposal area (including road/rail right-of-ways) 5. All facility -related structures and fences within the treatment, storage, and disposal areas 6. All habitable residences and places of assembly within 500 feet of all treatment, storage, and disposal sites 7. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds, ditches, and other surface drainage features within 500 feet of all waste treatment, storage, and disposal site(s) 8. Location and identification of each monitoring well (identify any background/upgradient wells) 9. Latitude and longitude coordinates of all monitoring wells (decimal degrees to 6 digits) 10. The wetted perimeter of all irrigation/infiltration fields with field names (named according to the approved permit) 11. The delineation of compliance and review boundaries C. Groundwater 1. Address the frequency exceedances of fecal coliform in MW-5. 2. The groundwater monitoring program for this permit needs to be re-evaluated. Monitoring well MW-5 has fecal coliform exceedances. It is unclear if MW-4 and MW-7 are within the compliance boundary. Typically, wells are installed on the review boundary. Please address the issues related to groundwater monitoring and groundwater standards. The Division recommends the Permittee schedule a meeting with the Washington Regional Office and Central Office to discuss. D. Lagoon 1. Provide the following information related to the lagoon: liner type (if there), liner full or partial, and date constructed.
