Dear Ms. Englestad:
On March 9, 2020, Brian Lambe from the Wilmington Regional Office of the Division of Energy, Mineral,
and Land Resources (DEMLR), conducted a compliance evaluation inspection for North Carolina Plywood
located in Columbus County, North Carolina. The site drains to a tributary of White Marsh, which is
currently classified as Class C, Sw Waters in the Cape Fear River Basin.
The following observations were noted during the DEMLR inspection (please see the attached
addendum for additional information about your permit):
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has not been developed, recorded, and properly
implemented. Please review permit requirements and strive to meet all documentation requirements.

Qualitative monitoring has not been conducted and recorded in accordance with permit requirements.

You should be able to record a qualitative monitoring event in the ditch semi annually.
3) Analytical Monitoring
Analytical monitoring has not been conducted and recorded in accordance with permit requirements.

If you cannot acquire a sample, you must document no flow for each semi annual event.

of the NPDES Stormwater General Permit may be subject to a civil penalty assessment of up to

Inspection Summary.
Greater attention in needed in maintenance of the permit and the SPPP Facility is down to only a few employees, from its
height of over 100. Not much industrial process occurs outside of cover and so the best best management practice BMP is
reduction of exposure One key area is the dust cyclones, as the excess dust can runoff in the stormwater.
Observed the lone outfall with the site manager. The outfall is the ditch in the corner of the property. He said that he can
never find a discrete flow that he can sample. The site sheet flows to the outer ditches. Suggest conducting qualitative
monitoring in the ditch if an analytical sample cannot be pulled.
