In accordance with your permit major modification request received June 25, 2019, and subsequent
additional information received September 9, 2019 and December 27, 2019, we are forwarding herewith
modified Attachments A and B certified March 16, 2020, to Terra Renewal Services, Inc. for the subject
residuals management program.
Modifications to the subject Attachments are as follows:
➢ Attachment A — Addition of 1,600 dry tons per year of residuals from the Tyson — Wilkesboro
facility, NC in Wilkes County; Reduction of 600 dry tons per year of residuals from River
Valley Animal Foods, Harmony, NC (Permit No. NC0005126) in Iredell County; reduction of
250 dry tons per year of residuals from the Hunter Farms High Point Facility in Guilford
County; addition of 160 dry tons per year from Brakebush Brothers, Mocksville, NC in Davie
County; addition of 5 dry tons per year of residuals from the Advance Pierre Foods Claremont
facility in Catawba County.
➢ Attachment B — Addition of a total of 16 fields totaling 490.2 acres; with 15 fields in Wilkes
County totaling 457.9 acres, those fields being NC-WK-DF-01 to -04, NC-WK-DS-01 to -10,
and NC-WK-WB-01; and 1 field in Yadkin County totaling 32.3 acres, that field being NC-
YK-TA-01.
The modified Attachments A and B shall be effective from the date of certification until January
31, 2021 (i.e., expiration date of the current permit), shall void Attachment A previously certified on June
29, 2018, shall void Attachment B of Permit No. WQ0038087 issued February 12, 2016, and shall be subject
to the conditions and limitations as specified therein, as well as those specified in the above -referenced
permit. Please replace Attachment A that was certified June 29, 2018 and Attachment B of Permit No.
WQ0038087 that was issued February 12, 2016, with the modified Attachments A and B. Please note that
the Permittee shall submit a renewal application no later than August 4, 2020.
Please note that only the maps for new land application sites are attached to this letter. It is the
Permittee's responsibility to ensure that a complete permit is created by inserting these documents into the
previously issued permit.

If any parts, requirements or limitations contained in this certification are unacceptable, the
Permittee has the right to request an adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.

2. Analyses to demonstrate that residuals are non -hazardous (i.e., TCLP, ignitability, reactivity, and corrosivity) as stipulated under permit Condition IV.2.
3. Testing of metals and nutrients as stipulated under permit Condition IV.3.
4. Analyses of pathogen and vector attraction reductions as stipulated under permit Condition IVA.
5. Monitoring frequencies are based on the actual dry tons applied per year using the table below, unless specified above.

If no land application events occur during a required sampling period (e.g., no land application occur during an entire year when annual monitoring is required), then no sampling data is
required during the period of inactivity. The annual report shall include an explanation for missing sampling data. Those required to submit the annual report to United States Environmental
Protection Agency (US EPA) may be required to make up the missed sampling. Contact the US EPA for additional information.
