Dear Mr. Carlton:
A letter of request for an Authorization to Construct was received November
23, 1988, by the Division and final plans and specifications for the subject project
• have been reviewed and found to be satisfactory. Authorization is hereby granted
for the construction of additional facilities to upgrade the existing treatment
facility to accomodate one million gallons per day consisting of modification of the
existing treatment plant into a aerobic sludge digestor, a 1150 GPM influent pump
station with triplex pumps, a 1.27 .million gallon aeration basin, three 1,400 scfm
blowers for aeration, a new 113,000 gallon circular clairfier, three parshall flumes
for sludge measurement, a digestor decanting pump station with dual 300 GPM pumps
with return of decanted wastewater back to the equalization basin, a additional
chlorine contact chamber bringing the total contact chamber volume to 26,670 gallons
and two groundwater monitoring wells with discharge of treated wastewater to Haw
Branch.
This Authorization to Construct is issued in accordance with Part III paragraph
B of the NPDES Permit No. NCO020338 issued December 10, 1987, and shall be
subject to revocation unless the wastewater treatment facilities are constructed in
accordance with the conditions and limitations specified in Permit No. NC0020338.
The Permittee must employ a certified wastewater operator in accordance with
Part III paragraph C of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G. S. 143-215. 1 and in a manner approvable by the North Carolina
Division of Environmental Management.
The Winston-Salem Regional Office, telephone number '919/761-2351 shall be
notified at least twenty-four (24) hours in advance of operation of these
. facilities. Such notification to the Regional Supervisor shall be made during the
normal office hour from 8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.

In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional
wastewater treatment and disposal facilities.
Upon completion of construction and prior. to operation of this permitted
facility, a certification must be received from a professional engineer certifying
that the permitted facility has been installed .in accordance with the approved plans
and specifications. Mail the Certification to the Permits and Engineering Unit,
P.O. Box 27687, Raleigh, N.C. 27611-7687.
A set of approved plans and specifications for the subject project must be
retained by the applicant for the life of the project.
The Town of Xadkinville shall submit an application for land application of
municipal sludge to the Division of Environmental Managment on or before August 1,
1989.
No sludge shall be returned to the equalization basin from the sludge decant
pump station.
Within 90 days of permit issuance, two (2) monitor wells, one (1) upgradient
and one (1) downgr.adi.ent, shall. be installed to monitor groundwater quality. The
l-ocAtion and construction details for these wells shall be approved by the
Winston-Salem Regional. Office, from which a well construction permit must be •
obtained.
The monitor wells must be sampled initially after construction and thereafter
every March, July, and November for the following parameters:
NO3 Ammonia Nitrogen
TDS Total Coliforms
TOC PH
Chloride TOX (in November only)
The mnasur.nment of water level must be made prior to sampling for the remaining
parameters.
If TOC concentrations greater than 3.0 mg/l are detected in any downgradient
monitoring wel.l., additional sampling and analysis must be conducted to identify the
individual constituents comprising this TOC concentration. If the TOC concentration
as measured in the background monitor well exceeds 10 mg/l, this concentration will
be taken to represent the naturally occuring TOC concentration. Any exceedances of
this natrrally occuring TOC concentration in the downgradient wells shall be subject
to the additional sampling and analysis as described above.

The analytical methods used for TOX must be capable of detecting the total of
all halogenated organic compounds present at a concentration of 5.0 parts per.
billion (ppb) or. greater. In the event that a TOX concentration of 5.0 ppb or
greater i.s detected, any individual halogenated organic compound(s) present at a
concentration at or above the method detection limit (MD1,) must be identified and
quantified utilizing EPA methods 601, 602, 604, and 611.
The results of the sampling and analysis shall be sent to the N.C. Division of
Environmental. Management on Form GW-59 (Compliance Monitoring Report Form) every
April, August and December
Any additional groundwater quality monitoring as deemed necessary 'by the
Department, shall be provided.
