On behalf of Edgecombe County, we are sending you the following documents for your
review and approval of the above - referenced project. The project will eliminate failing
residential septic systems in the Logsboro Road/NC 33 Highway area. Proposed
improvements are all located within existing, previously disturbed rights -of -way.
Encroachments into Zone 1 and 2 buffers will be limited to previously disturbed locations
and no new disturbance is anticipated. The following files are included with this application:

3. Project Description
3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
application:
The existing conditions of the site are graded roadside ditches and private lanes. The proposed gravity sewer will be
placed at the previously disturbed right -of -way line with permanent easements throughout the project. Where no right -of -
way is present, permanent easements will be acquired by the County to install and maintain the new gravity sewer lines.
The general land uses in the vicintiy of the project are opentcultivated fields and single home residences within the project
area.

3d. Explain the purpose of the proposed project:
The project is proposed to provide public sanitary sewer to the residents along NC Highway 33 and Logsboro Road and
the adjacent private lanes, and eliminate failing residential septic systems.
3e. Describe the overall project in detail, including the type of equipment to be used:
The project will involve the construction of two (2) pump stations, approximately 18,700 LF of 8 -inch gravity sewer, and
approximately 15,000 LF of 64nch force main. The gravity sewer and pump stations will be installed using trackhoes and
other related equipment to dig the trenches within the proposed right -of -ways and permanent utility easements. Proper
erosion control measures will be used to prevent sediment moving offsite. The one irrigation ditch (deemed an
intermittent stream) will be open cut but will use proper measures to prevent downstream impacts.

6i. Comments: The buffer impacts are shown on sheets C.2.2 and C.2.14. Buffer impacts identification labels have been
added to the plan set. The majority of the buffer impacts are in non - NCDOT existing transporation facility, which previously
disturbed and presently maintained as transportation infrastructure. Given the areas are previously disturbed and will require
no clearing or grubbing, mitigation is not anticipated. Impacts will be temporary in nature and will be minimized to the greatest
extent possible while using sedimentation and erosion control measures. The alignment of proposed lines was designed to
minimize disturbance to the greatest extent practical.

Most of the sewer lines will be constructed in previously disturbed land adjacent and within the NCDOT right -of -way. Impacts
will be minimized by using proper sedimentation and erosion control measures and by limiting the amount of disturbed area to
what is shown on the plans. Trench boxes are also anticipated to reduce disturbed area.

Minimizing trench width, minimizing sediment runoff by using sedimentation and erosion control measures, and immediate
seeding and mulching.

A. DESCRIPTION OF PROJECT
Location of Project
The proposed sanitary sewer project is located along NC Highway 33 and Logsboro Road
within Edgecombe County Water and Sewer District No. 5 between the Tar River and
Community of Leggett as well as a small section of force main along NC Highway 97 inside
the corporate limits of the Town of Leggett. This sewer project will be owned by
Edgecombe County's Water and Sewer District No. 5, where it resides. The majority of the
work will take place within previously disturbed NCDOT right -of -ways and private utility
easements along New Logsboro Road and NC Highway 33 in addition to the community
around Jerys Mobile Home Park (MHP) and Forest Pine Drive MHP. A vicinity map has
been attached which shows the exact project location in Edgecombe County.
Type of Development
Gravity Sewer
The gravity portion of the proposed sanitary sewer project will be a linear construction
project consisting of installing approximately 18,700 LF of 84nch gravity sewer and 76
sanitary sewer manholes.
The proposed network will be divided into two (2) distinct sewersheds. The first sewershed
is located in the southern portion of the project area including Jerry's MHP, Forest Pine
Drive MHP and Ellis Lane. The collected wastewater will be directed to the Highway 33
Pump Station. This sewershed will included installing approximately 10,550 LF of 8 -inch
gravity sewer and 45 sanitary sewer manholes.
The second sewershed is located in the central portion of the project area including
Logsboro Road and Boyd Lane. The collected wastewater will be directed to the Logsboro
Road Pump Station. This sewershed will included installing approximately 8,150 LF of 8-
inch gravity sewer and 31 sanitary sewer manholes.
Pumu Station & Force Main
The collected wastewater in the southern sewershed will be conveyed to the new Highway
33 Pump Station, which is located on a parcel owned by Edgecombe County adjacent to
NC Highway 33 near Forest Pine Drive. The wastewater will then be pumped through
approximately 14,950 LF of a new 6 -inch force main to the existing Manhole No. 44 which
is part of the Town of Leggett wastewater collection system. The existing downstream
network will convey the wastewater to the City of Rocky Mount's Wastewater Treatment
Facility (WWTF).
The Highway 33 Pump Station is designed to pump at 180 GPM at 87' TDH, which will
maintain greater than 2 feet per second velocity in the receiving force main.
Likewise, the collected wastewater in the northern sewershed will be conveyed to the new
Logsboro Road Pump Station, which is located on a parcel owned by Edgecombe County
adjacent to NC Highway 33 and W. Logsboro Road. The wastewater will then be pumped
through approximately 50 LF of a new 6 -inch force main to the proposed 64nch force main
from the Highway 33 pump station and continue pumping to the existing Manhole No. 44
which is part of the Town of Leggett wastewater collection system. The existing
downstream network will convey the wastewater to the City of Rocky Mount's Wastewater
Treatment Facility (WWTF).
The Logsboro Road Pump Station is designed to pump at 180 GPM at 78' TDH, which will
maintain greater than 2 feet per second velocity in the receiving force main.

Earth Disturbing Activities
The project will involve digging trenches for the 8 -inch gravity sewer lines, the new 6 -inch
force mains, and the construction of the Highway 33 and Logsboro Road Pump Stations.
The width of the installation trench will vary based on the depth of the gravity sewer. At the
deepest parts, the disturbed area will be approximately 40 feet wide including the spoil pile.
One drainage ditch (deemed to be an intermittent unnamed tributary to Swift Creek) will be
open cut to install the 8 -inch gravity sewer that is proposed for that location. Sedimentation
and erosion control measures will be used throughout the project, but most importantly at
the main stream crossing and at the many small ditch crossings. The required erosion
control measures will be installed prior to construction. A track -hoe will be used in the
excavation process with other associated equipment for sewer system installation. To
cross NCDOT roads, a process of boring and jacking will be utilized.
Existing Drainage Patterns
Edgecombe County is generally broad and flat sloping from west to the east. The County
is located in the Tar - Pamlico River Basin and within Fishing Creek, Little Fishing Creek,
Rocky Swamp and Beech Swamp Subbasin (03 -03 -04). The majority of the area located
within this project area drains to road side ditches and /or unnamed tributaries, which
ultimately flow to Swift Creek. Swift Creek flows directly to the Tar River that bisects the
County.
B. EROSION AND SEDIMENTATION CONTROL MEASURES
1. A copy of the latest approved sediment and erosion control plan must be on file at the job
site. Timely and proper attention shall be required for prevention of offsite erosion and
sedimentation. Various levels of erosion control are proposed in the engineering
documents. If it becomes evident that additional erosion and sedimentation control
measures are needed they shall be installed immediately by the contractor. A temporary
cover sufficient to restrain erosion must be provided within the 21 calendar days of
completion of any phase of land disturbance. A permanent ground cover, sufficient to
restrain erosion, must be provided within the shortest of 14 working or 90 calendar days
after completion of the construction or development.
2. A buffer zone, sufficient to restrain sedimentation, must be maintained between the land -
disturbing activity and any adjacent property or watercourses. Maintain / Do Not Disturb
existing vegetative cover in areas of no construction.
3. All disturbed areas will be seeded within 30 days following construction in accordance
' with the seeding schedule as shown on the plans. We have attached the seeding and
mulching specifications to this document which gives the rates for fertilizer, tack, seeding
and mulch.

4. Temporary sediment control primarily will consist of temporary check dams and arc filters
which will be placed along the existing roadside ditches. The rock check dams shall be a
maximum of 1,000 LF apart and the volume of storage shall accommodate this sediment
as shown on the engineering documents. In the proposed project area, the slopes along
most of the roadside ditches are so flat that the spacing allows for a great amount of
storage before over flowing into the next drainage system.
In order to eliminate erosion from traveling downstream, arc filters will be utilized when
sewer lines bisect drainage culverts on the upstream side of the culvert.
5. Silt fence will be utilized in areas next to the blue line stream to prevent erosion of the
disturbed soil for the sewer line crossing. In the location of ditch crossings, rip rap is to be
' placed at the opening of the storm drainage pipe to prevent erosion of the disturbed soil
for the pipeline crossing. Silt fence shall also be used to prevent erosion in areas where
bore and recovery pits are to be constructed and at open culvert crossings the silt fence
is to be tied into the temporary sediment traps or check dams as shown in the
' engineering documents. Silt fence will also be utilized at the pump station site. Since the
velocity of flow through the roadside ditches is less than 2.5 fps, lining is not required but
there may be areas where the contractor will be required to place matting if drainage
' ditch disturbance occurs.
6. If the contractor chooses to place any of the material on the adjacent roadway the
contractor shall be required to place screening over the roadway surface prior to
' beginning the operations to assist in cleaning up the roadways after construction.
C. TIME SEQUENCE OF OPERATIONS
Construction is anticipated to begin February 2014. Permits are being secured now and
' once the project is awarded, construction will be able to start. The contractor is required
under the contract to comply with all state regulations regarding erosion and
sedimentation control. Once the sediment and erosion control measures are in place, the
contractor will begin installing the sewer lines, manholes, and pump stations. The
' contractor shall complete all construction within the easements and rights -of -ways as
shown on the plans. All utilities must be checked and tested and any areas that can be
seeded should be done as soon as possible.
' Once the subsurface infrastructure is complete and tested, the contractor shall finish
grading and seeding remaining areas. Seed bed preparation details have been added to
the drawings. Hydro-seeding will not be used on this project. In areas where the trench
' line is within the bottom of a ditch, erosion control matting shall be used to stabilize the
soil.
' The erosion and sediment control measures are to be checked weekly and after any
rainfall event. Repairs shall be made immediately to the sediment and erosion control
measures as needed during the project.

Map Unit Descriptions
The map units delineated on the detailed soil maps in a soil surrey represent the soils
or miscellaneous areas in the survey area. The map unit descriptions, along with the
maps, can be used to determine the composition and properties of a unit.
A map unit delineation on a soil map represents an area dominated by one or more
major kinds of soil or miscellaneous areas. A map unit is identified and named
according to the taxonomic classification of the dominant soils. Within a taxonomic
class there are precisely defined limits for the properties of the soils. On the landscape,
however, the soils are natural phenomena, and they have the characteristic variability
of all natural phenomena. Thus, the range of some observed properties may extend
beyond the limits defined for a taxonomic class. Areas of soils of a single taxonomic
class rarely, if ever, can be mapped without including areas of other taxonomic
classes. Consequently, every map unit is made up of the soils or miscellaneous areas
for which it is named and some minor components that belong to taxonomic classes

Most minor soils have properties similar to those of the dominant soil or soils in the

In the November 4, 1997 Federal Register (55822 - 55825), the northern
tpopulation of the bog turtle (from New York south to Maryland) was listed as T
(threatened), and the southern population (from Virginia south to Georgia) was

listed as T(S /A) (threatened due to similarity of appearance). The T(S /A)
' designation bans the collection and interstate and international commercial
Evade of bog turtles from the southern population. The T(S /A) designation has
no effect on land management activities by private landowners in North
' Carolina, part of the southern population of the species. In addition to its
official status as T(S /A), the U.S. Fish and Wildlife Service considers the
' southern population of the bog turtle as a Federal species of concern due to
habitat loss.

' On February 2, 2012, we received notification from the State Clearinghouse concerning the above project.
' There are no known archaeological sites within the proposed project area. Based on our knowledge of the
area, it is unlikely that any archaeological resources that may be eligible for inclusion in the National Register of
Historic Places will be affected by the project. We, therefore, recommend that no archaeological investigation
' be conducted in connection with this project.
A portion of this project is located adjacent to Cedar Lane, which is listed in the National Register of Historic
Places. However, since the project will be located within the existing right -of -way, and will not require the
' removal of mature landscaping, the proposed project will have no adverts f cl on historic properties.
The above comments are made pursuant to Section 106 of the National Historic Preservation Act and the
' Advisory Council on Historic Preservation's Regulations for Compliance with Section 106 codified at 36 CFR

the Floodway of Swift Creek, approval of either a no -rise study with a no -rise

Map Revision, will be required prior to construction.

This letter is to inform you that a list of all federally- protected endangered and threatened species
with known occurrences in North Carolina is now available on the U.S. Fish and Wildlife
Service's (Service) web page at http: / /www.fws.gov /raleigh. Therefore, if you have projects that
occur within the Raleigh Field Office's area of responsibility (see attached county list), you no
longer need to contact the Raleigh Field Office for a list of federally- protected species.

Our web page contains a complete and frequently updated list of all endangered and threatened
'
species protected by the provisions of the Endangered Species Act of 1973, as amended (16
U.S.C. 1531 et seq.)(Act), and a list of federal species of concern' that are known to occur in
each county in North Carolina.
Section 7 of the Act requires that all federal agencies (or their designated non - federal
representative), in consultation with the Service, insure that any action federally authorized,
funded, or carried out by such agencies is not likely to jeopardize the continued existence of any
federally - listed endangered or threatened species. A biological assessment or evaluation may be
p%pared to fulfill that requirt'mt -nt and in determining whether additional consultation with the

Service is necessary. In addition to the federally- protected species list, information on the
species' life histories and habitats and information on completing a biological assessment or
evaluation and can be found on our web page at http: / /www.fws.gov /raleigh. Please check the

web site often for updated information or changes.

The term "federal species of concern" refers to those species which the Service believes might be in need of
concentrated conservation actions. Federal species of concern receive no legal protection and their designation does
'
not necessarily imply that the species will eventually be proposed for listing as a federally endangered or threatened
;pecies. However. we recommend that all practicable measures be taken to avoid or minimize adverse impacts to
federal species of concern_

If your project contains suitable habitat for any of the federally- listed species known to be
' present within the county where your project occurs, the proposed action has the potential to
adversely affect those species. As such, we recommend that surveys be conducted to determine
the species' presence or absence within the project area. The use of North Carolina Natural
' Heritage program data should not be substituted for actual field surveys.
If you determine that the proposed action may affect (i.e., likely to adversely affect or not likely
' to adversely affect) a federally - protected species, you should notify this office with your
determination, the results of your surveys, survey methodologies, and an analysis of the effects
of the action on listed species, including consideration of direct, indirect, and cumulative effects,
' before conducting any activities that might affect the species. If you determine that the proposed
action will have no effect (i.e., no beneficial or adverse, direct or indirect effect) on federally
listed species, then you are not required to contact our office for concurrence (unless an
' Environmental Impact Statement is prepared). However, you should maintain a complete record
of the assessment, including steps leading to your determination of effect, the qualified personnel
conducting the assessment, habitat conditions, site photographs, and any other related articles.
With regard to the above - referenced project, we offer the following remarks. Our comments are
submitted pursuant to, and in accordance with, provisions of the Endangered Species Act.
Based on the information provided and other information available, it appears that the proposed
action is not likely to adversely affect any federally- listed endangered or threatened species, their
' formally designated critical habitat, or species currently proposed for listing under the Act at
these sites. We believe that the requirements of section 7(a)(2) of the Act have been satisfied for
your project. Please remember that obligations under section 7 consultation must be
reconsidered if (1) new information reveals impacts of this identified action that may affect
listed species or critical habitat in a manner not previously considered; (2) this action is
subsequently modified in a manner that was not considered in this review; or, (3) a new species
' is listed or critical habitat determined that may be affected by the identified action.
However, the Service is concerned about the potential impacts the proposed action might have
' on aquatic species. Aquatic resources are highly susceptible to sedimentation. Therefore, we
recommend that all practicable measures be taken to avoid adverse impacts to aquatic species,
including implementing directional boring methods and stringent sediment and erosion control
' measures. An erosion and sedimentation control plan should be submitted to and approved by
the North Carolina Division of Land Resources, Land Quality Section prior to construction.
Erosion and sedimentation controls should be installed and maintained between the construction
' site and any nearby down - gradient surface waters. In addition, we recommend maintaining
natural, vegetated buffers on all streams and creeks adjacent to the project site.
' The North Carolina Wildlife Resources Commission has developed a Guidance Memorandum (a
copy can be found on our website at (http: / /www.fws.gov /raleigh) to address and mitigate
secondary and cumulative impacts to aquatic and terrestrial wildlife resources and water quality.
' We recommend that you consider this document in the development of your projects and in
completing an initiation package for consultation (if necessary).
