In accordance with your permit major modification request received December 3, 2019, and
subsequent additional information received January 31, 2020, we are forwarding herewith a modified
Attachment A certified March 9, 2020, to Bio-Nomic Services, Inc. for the subject residuals management
program.
Modifications to the subject Attachment are as follows: Addition of 2,000 dry tons/year from
George L. Bernhardt, Sr. Lake Rhodhiss WTP (Permit No. NCO114010) located in Caldwell County.
The modified Attachment A shall be effective from the date of certification until February 28, 2022
(i.e., expiration date of the current permit), shall void Attachment A of Permit No. WQ0030953 issued
March 22, 2017 that was previously certified on April 16, 2019, and shall be subject to the conditions and
limitations as specified therein, as well as those specified in the above -referenced permit. Please replace
Attachment A of Permit No. WQ0039053 that was certified on April 16, 2019 with the modified
Attachment A. Please note that the Permittee shall submit a renewal application no later than
September 1, 2021.
If any parts, requirements or limitations contained in this certification are unacceptable, the
Permittee has the right to request an adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.
