On November 23, 2009 the Division of Water Quality (DWQ) received your application sets for the above
referenced project. The DWQ has determined that your application was incomplete and/or provided inaccurate
information as discussed below.
Additional Information Requested
The PCN Application failed to contain the Applicant/Agent's Signature and Date. See last page of
application. Please visit the following website, print off only the signature page, mail in one (1) original and
four (4) copies to begin the review and approval process. DWQ is unable to accept faxed or emailed copies for
legal purposes. Please attach a copy of this email so we are able to match the signature pages with your
project.
