Larkins Water Pollution Control Facility, Certificate of Coverage NGG110039 Sampson County Dear Mr. Medlin: On February 27. 2020, a site inspection was conducted for the Norman H. Larkins Water Pollution Control facility located at 123 Mill Branch Road, Sampson County, North Carolina. A copy of the Compliance Inspection Report is enclosed for your review. Ms. Lisa Osthues, Environmental Programs Manager, Mr. Blake Raynor, Chief Wastewater Operator and Mr. Neil Carroll, Superintendent were also present during the inspection and their time and assistance is greatly appreciated. The site visit and file review revealed that the subject facility is covered by NPDES Stormwater General Permit NCG110000 under Certificate of Coverage NCG110039. Permit coverage authorizes the discharge of stormwater from the facility to receiving waters designated as Williams Old Mill Branch, a Class C waters in the Cape Fear River Basin. As a result of the inspection, the facility was found to be compliant with the conditions of the NCG110000 permit. Please refer to the enclosed Compliance Inspection Report for additional comments and observations made during the inspection. Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil penalty assessment of up to $25,000 per day for each violation. The site map should be updated to -include the location of the Maintenance building. A feasability study was not included in the SPPP. The semi-annual facility inspections should include inspections of all Best Management Practices being utilized on the grounds of the facility (e.g. ditches and riprap protection, etc.) These measures were inspected as well as Outfall 1. There was erosion noted downslope of this outfall. A repair plan has been developed for this issue. There was also erosion observed undercutting three culverts discharging to a ditch. Near the stormwater pumps, erosion was observed near.a drop inlet which needed additional protective stabilization. The facility grounds appeared well -maintained.
