Dear Mr. Raulerson: '
We have.been notified that on November•17, 2016, you submitted samples for analysis of BOD and
Suspended Residue to Environmental Testing Solutions, Inc. At your request the samples .were •
analyzed without proper preservation. Samples were > 6 °C upon receipt by the laboratory. Please note
that composite samples are required to be refrigerated •or•. iced during collection and during
transportation

Please•be advised.that under provisions of North Carolina Regulation,-15A'NCAC .0805 (a) (7) (N), any
time a laboratory receives samples that do not meet sample collection, holding time, or preservation
requirements, the laboratory must notify the--sample collector 'or client and secure another sample if
possible. If another sample cannot be secured, the original sample may be analyzed but the results
reported.(on both the client report and the Discharge. Monitoring Report or other regulatory compliance
report submitted to the state'of'North Carolina) must bequalified with the nature of the infraction(s), and
the laboratory must notify the State Laboratory about the infraction(s). The notification must include a
statement indicating corrective actions taken to prevent the problem for future samples.
Please 'respond to this office within 15 days regarding the steps taken'to prevent recurrence in the
future: Failure to comply with these requirements may-result in enforcement action..

Pertaining to our cohversation on Tuesday, November 29, 2016, starting with the
sample on November 30, 2016, I logged the temperature of the Effluent water
sample for Royal Oaks, Inc, on the chain of custody form. I will make a habit to do
that'in the future. Today's temperature was 7 deg. Celsius. Refrigerator
temperature was 5 deg. Celsius.

Please be advised that Environmental Testing Solutions, Inc has analyzed BOD and Suspended
Residue samples, at the client's request, which were not properly preserved. These samples were
for the Royal Oaks facility. Per 15A NCAC 2H .0805 (a) (7) (N) any time a sample is received which .
does not meet sample collection, holding time, or preservation requirements, the laboratory must notify
the sample collector or client and secure another sample if'possible. If another sample cannot be
secured, the original sample may be analyzed but.the results reported must be qualified with the nature
of the infraction(s) and the laboratory must notify the State Laboratory about the infraction(s).

I am not sure if you and I talked about this;Jason said he spoke with you about this. I am forwarding this to you as an
FYI at this point. Keep it on your radar next time you inspect this facility or any facility associated with this operator.This
way you can look into this/discuss with the operator as a follow up.
We are also going to talk further about this at the next WW Team mtg to see how best to proceed with these situations
on our end.
