I visited the Appalachian Stone Maintenance Facility on February 27, 2020. 1 met with Daniel Rey on site. Based on the
evaluation of the NOI, I informed Mr. Ray that the NOI would need to be resubmitted with appropriate coordinates, and
signed. He is aware, and should be submitting a revised NOI soon. Therefore, I recommend you deny coverage as
proposed, until a corrected NOI has been submitted.

This NOI is for the facility associated with NOV-2019-PC-0679. They were sent an NOV for operating without a permit.
