Dear Mr. Prostinak:
A letter of request for an Authorization to Construct was received January 4,
1987, by the Division and final plans and specifications for the subject project have
been reviewed and found to be satisfactory. Authorization is hereby granted for the
construction and operation of a flow equilization type structure consisting of 150
linear feet of 5-feet diameter reinforced concrete pipe to serve as alternate means of
holding surge flows from influent pump station No. 11 for the Town of Lake Waccamaw
Wastewater Treatment Plant located in Columbus County. This authorization also allows
the construction of 55 linear feet of_$-inch and 50 linear feet of 12-inch pipe and
all related appurtenances to connect the proposed structure with existing facilities.
This ,Authorization to Construct is issued in accordance with Part III paragraph C
--- of the NPDES Permit No. NC0021881 issued July 1, 1987, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NC0021881.
_ The Wilmington Regional Office, telephone number 919/256-4161 shall be notified
at- le-ast twenty-four (24) hours in advance of operation of the installed system so
that in-place inspection can be made. Such notification to the Regional Supervisor
shall be made during--the normal office hours--from 8:00 a.m. until 5:00 p.m. on Monday
thorough Friday, excluding State Holidays.
In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the Permittee shall take such immediate corrective action as
_may be required by this Divisions including- the_consaruction of additional wastewater--
collection treatment- and disposal facilities.

The infiltration/exfiltration test rate shalt not exceed 200 GPD per
Inch-diameter mile of pipe.
Solids, sludges, or other pollutants removed or resulting from the wastewater
storage facilities shall be contained and disposed of in such a manner as to
prevent any contamination of the surface or groundwater of the State.
Upon completion of construction and prior to .operation of this permitted flow
equalization facility, a certification must be received from a professional engineer
certifying that the permitted facility has been installed in accordance with the
approved plans and specifications.
