Dear Mr.Edwards:
I am in receipt of the NOTICE OF VIOLATION NOV-2009-PC-0389. Please accept this as a
partial explanation of the issues with a more detailed explanation to come in the next week or
two. The Town of Murphy is in the process of developing a local pretreatment permit for
Carolina Prime Pet. I will be forwarding you a copy within the next couple of weeks with the
details. We have also fined Carolina Prime Pet for their violation.
We have had several electrical problems that have resulted in various problems such as motors
being ruined. This is something we have not taken lightly but have had an extremely difficult
time in identifying our problems. We have had Murphy Power Board conduct an"audit"but with
no results. We contacted Mike Waresak with McGill Associates to assist us by sending an
electrical engineer (Phil Fisher) to the plant to see if they could help identify or resolve our
problems. Upon their recommendation, we have contracted with Square D (GraybaR in
Asheville) to do a complete analysis of our system. I am attaching a copy of the proposed work
to be done.
We are working diligently to correct our problems at the plant. I will update you as soon as I
have information.

Proposal for Engineering Services Quote#LHO42309A
This is a proposal for services to be performed for Town of Murphy in Murphy,NC,by Square D Company.
1 Scope of Work
Square D Engineering Services proposes to install two temporary monitors to characterize the power quality on
the equipment panels that are experiencing the most problems. System RMS voltages and currents, as well as
other steady-state parameters such as harmonic distortion and unbalance levels, will be recorded. Power
quality disturbances such as voltage sags will also be monitored and recorded. All data collected will be
compared to industry standard limits. Recommendations for corrective action will be provided as needed. The
following tasks will be performed.
Y Interview facility personnel to assess the history of problems with sensitive equipment.
• Review of the existing distribution system. Inspect wiring and grounds.
0 Two weeks of temporary power monitoring at the panels using Powedogic PCM4000T Monitors.
2 Cost of Services
The price for these services is$4,500.00 user net. This quote is valid for 30 days.
3 Clarifications to Scope
• A detailed summary of analysis and recommendations will be provided in a written report.
• All engineering analysis to be performed or supervised by a registered professional engineer.
• Square D Power System Engineer to spend up to one day on sight to install temporary monitor and collect
any data necessary for power quality study. At the end of the monitoring period,Squared D to remove and
collect monitor.
4 Schedule
Work can begin within two weeks after receipt of order. The final report will be delivered within two weeks after
collection of all required data.
5 Warranty
Square D warrants that the services provided will meet industry standards. The warranty remains in effect for
one year from the completion of the work and is further explained in the Square D Field Services'Terms and
Conditions of Supply and Performance."
6 Terms and Condtions
The work described in this proposal will be performed in accordance with Square D Field Services'Terms and
Conditions of Supply and Performance, dated January, 2001, which are attached to this proposal. No other
terms and conditions shall apply and Square D specifically rejects any preprinted terms and conditions on
customers purchase order.
Square D must be granted ready access to the equipment under investigation. Square D will need access to
the facility's electrical panels, and the assistance of an electrician or facilities technician familiar with the
electrical circuits and loads. Square D reserves the right to discontinue any work deemed by Square D to be
unsafe. All on-site work will be performed in strict accordance with NFPA 70E, Standard for Electrical Safety in
the Workplace.
