Division of Water Resources' Central and Regional staff has reviewed the application package
received December 4, 2019. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on March 25,
2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items,
or failure to provide the additional information on or before the above requested date may result in your
application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy.

A. Application (15A NCAC 02T .0105):
1. The facility is on property owned by Perdue Real Estate Holdings, Inc. Provide legal documentation
verifying the Permittee (Perdue Foods, LLC) owns or is the same entity as the property owner. Or
provide an easement meeting the requirements of 15A NCAC 02L .0107.
2. Provide a detailed industrial process description including:
a. Overview of the wastewater generation process;
b. Inventory of all materials used in the industrial process, including chemical and biological matter;
and
c. Overview of cleaning and treatment methodology, including an inventory of treatment materials
and compounds.
B. Site Map:
Provide an updated site map in accordance with 15A NCAC 02T .0105(d) that shows the following:
1. Legend, north arrow, and scale
2. Distance measurements verifying all setbacks are being met
3. Stormwater drainage controls
4. Location and ownership of property boundaries within 500 feet of the disposal area (including road/rail
right-of-ways)
5. All facility -related structures and fences within the treatment, storage, and disposal areas
6. All habitable residences and places of assembly within 500 feet of all treatment, storage, and disposal sites
7. Topographic contour intervals not exceeding 10 feet or 25 percent of total site relief
8. Soil mapping units on all disposal sites
9. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds, ditches, and
other surface drainage features within 500 feet of all waste treatment, storage, and disposal site(s)
10. Location and identification of each monitoring well (identify any background/upgradient wells)
11. Latitude and longitude coordinates of all monitoring wells (decimal degrees to 6 digits)
12. Location and identification of major components of the waste disposal system
13. Identification of all storage or treatment earthen structures that are part of the permit
14. The wetted perimeter of all irrigation/infiltration fields with field names (named according to the approved
permit)
15. The delineation of compliance and review boundaries
16. 100-year floodplain (if applicable)
C. Groundwater (15A NCAC 02L):
The groundwater monitoring program for this permit needs to be re-evaluated. Monitoring well MW-
3 has ammonia exceedances. It is unclear if MW-4 is within the compliance boundary. Typically,
wells are installed on the review boundary. Please address the issues related to groundwater monitoring
and groundwater standards. The Division recommends the Permittee schedule a meeting with the
Raleigh Regional Office and Central Office to discuss.
