The attachments should be clearly identified and presented in the order below to expedite review of the permit application package. 1. INJECTION ZONE — Per 15A NCAC 02C .0225(e)(2), specify the horizontal and vertical portion of the subsurface within which the proposed injection activity will take place and beyond which no violations of groundwater quality standards shall result from the injection as determined by an approved monitoring plan. The determination shall be based on the hydraulic properties of the specified zone. Provide any supporting documentation in a separate attachment. 2. HYDROGEOLOGIC EVALUATION — Per 15A NCAC 02C .0225(e)(3), provide a hydrogeologic evaluation of the injection zone that includes all of the following: (A) Regional and local geology and hydrology; (B) Changes in lithology underlying the facility; (C) Depth to bedrock; (D) Depth to the mean seasonal high water table; (E) Hydraulic conductivity, transmissivity, and storativity, of the injection zone based on tests of site -specific material, including a description of the test(s) used to determine these parameters; (F) Rate and direction of groundwater flow as determined by predictive calculations or computer modeling; and (G) Lithostratigraphic and hydrostratigraphic logs of any existing test and injection wells. 3. INJECTANT INFORMATION — Per 15A NCAC 02C .0225(e)(5), provide information on each inj ectant as indicated below: (A) MSDS, concentration at the point of injection, and percentage if present in a mixture with other injectants; (B) The source of fluids used to dilute, carry, or otherwise distribute the injectant throughout the injection zone. If any well within the area of review of the injection facility is to be used as the fluid source, then the following information shall be submitted: location/ID number, depth of source, formation, rock/sediment type, and a chemical analysis of the water from the source well, including analyses for all contaminants suspected or historically recognized in soil or groundwater on the site; (C) A description of the rationale for selecting the injectants and concentrations proposed for injection, including an explanation or calculations of how the proposed injectant volumes and concentrations were determined; (D) A description of the reactions between the injectants and the contaminants present including specific breakdown products or intermediate compounds that may be formed by the injection; (E) A summary of results if modeling or testing was performed to investigate the injectant's potential or susceptibility for biological, chemical, or physical change in the subsurface; and (F) An evaluation concerning the development of byproducts of the injection process, including increases in the concentrations of naturally occurring substances. Such an evaluation shall include the identification of the specific byproducts of the injection process, projected concentrations of byproducts, and areas of migration as determined through modeling or other predictive calculations. 4. INJECTION PROCEDURE — Per 15A NCAC 02C .0225(e)(6), submit a table with a detailed description of the proposed injection procedure that includes the following: (A) The proposed average and maximum daily rate and quantity of injectant; (B) The average maximum injection pressure expressed in units of pounds per square inch (psi); and (C) The total or estimated total volume to be injected. FRACTURING PLAN (if applicable) — Per 15A NCAC 02C .0225(e)(7). submit a detailed description of the fracturing plan that includes the following: (A) Material Safety Data Sheets of fracturing media including information on any proppants used; (B) a map of fracturing well locations relative to the known extent of groundwater contamination plus all buildings, wells, septic systems, underground storage tanks, and underground utilities located within the Area of Review; (C) a demonstration that buildings, wells, septic systems, underground storage tanks, and underground utilities will not be adversely affected by the fracturing process; (D) injection rate and volume; (E) orientation of bedding planes, joints, and fracture sets of the fracture zone; (F) performance monitoring plan for determining the fracture well radius of influence; and (G) if conducted, the results of geophysical testing or pilot test of fracture behavior conducted in an uncontaminated area of the site. 6. WELL CONSTRUCTION DETAILS — Per 15A NCAC 02C .0225(e)(8). submit the following information in tabular or schematic form as appropriate for each item: (A) number and depth of injection wells; (B) number and depth of borings if using multi -level or "nested" well systems; (C) indication whether the injection wells are existing or proposed; (D) depth and type of casing; (E) depth and type of screen material; (F) depth and type of grout; (G) indication whether the injection wells are permanent or temporary "direct push" points; and (H) plans and specifications of the surface and subsurface construction details. 7. MONITORING PLAN — Per 15A NCAC 02C .0225(e)(9). submit a monitoring plan that includes the following: (A) target contaminants plus secondary or intermediate contaminants that may result from the injection; (B) other parameters that may serve to indicate the progress of the intended reactions; (C) a list of existing and proposed monitoring wells to be used; and (D) a sampling schedule to monitor the proposed injection. Monitoring wells shall he of sufficient quantity and location to detect any movement of injection fluids, injection process byproducts, or formation fluids outside the injection zone. The monitoring schedule shall he consistent with the proposed injection schedule, pace of the anticipated reactions, and rate of transport of the injectants and contaminants. 8. WELL DATA TABULATION — Per 15A NCAC 02C .0225(e)(10). provide a tabulation of data on all existing or abandoned wells within the area of review of the injection well(s) that penetrate the proposed injection zone, including monitoring wells and wells proposed for use as injection wells. Such data shall include a description of each well's type, depth, and record of construction or abandonment. 9. MAPS AND CROSS -SECTIONS — Per 15A NCAC 02C .0225(e)(11), provide scaled, site -specific site plans or maps depicting the location, orientation, and relationship of facility components including the following: (A) area map based on the most recent USGS 7.5' topographic map of the area, at a scale of 1:24,000 and showing the location of the proposed injection site; (B) topographic contour intervals showing all facility related structures, property boundaries, streams, springs, lakes, ponds, and other surface drainage features; (C) all existing or abandoned wells within the area of review of the wells listed in the well data tabulation that penetrate the proposed injection zone; (D) potentiometric surface map(s) that show the direction of groundwater movement, existing and proposed wells; (E) contaminant plume map(s) with isoconcentration lines that show the horizontal extent of the contaminant plume in soil and groundwater, and existing and proposed wells; (F) cross-section(s) to the known or projected depth of contamination that show the horizontal and vertical extent of the contaminant plume in soil and groundwater, major changes in lithology, and existing and proposed wells; and (G) any existing sources of potential or known groundwater contamination, including waste storage, treatment, or disposal systems within the area of review of the injection well or well system. INJECTION ZONE The area in which the injections are to occur is shown on the attached figure 5. The vertical extent of the injection activities is proposed for the water table aquifer situated at approximately 6 feet below grade to a depth of approximately 35 feet below grade. The injections are only proposed to be conducted on the site property. There are no public or private wells situated in the area of the injections. Based on a pumping test conducted on one of the monitor wells with a screened interval situated between 20 and 30 feet below grade, the groundwater velocity was determined to be approximately 0.5 feet per day. Based on the nature of the sodium lactate (food grade material), the longevity of the injectant in the groundwater is short term (approximately 1 year) before it is broken down and consumed by indigenous bacteria. 2. HYDROGEOLOGIC EVALUATION A) The Site is located in the Coastal Plain Physiographic Province and is underlain by a surficial aquifer formation, the Castle Hayne, and Peedee Formations. The surficial aquifer is unconfined and consists of interbedded gray, tan and brown sands and silts. The Castle Hayne confining unit separates the surficial aquifer from the Castle Hayne Aquifer. The top of the confining unit was encountered at approximately 40 feet below ground surface (bgs) at the site. The deepest boring to 79 feet bgs did not fully penetrate the Castle Hayne confining unit. The general groundwater flow direction across the site is to the north- northwest. B) Mostly gray and tan fine sand underlie the site from ground surface to approximately 40 feet bgs. A five-foot thick layer of light grey, calcareous clayey silt and limestone fragments was encountered between 40 and 45 feet bgs, marking the top of the Castle Hayne confining unit grading to green marine silt which was documented to 79 feet bgs by the deepest boring, MW-WSP-2XD. (C) Depth to Rock: undetermined (D) Depth to mean seasonal water table: 4 to 6 feet bgs. (E) Hydraulic Conductivity: 54 feet per day Transmissivity: 11,300 gallons per day per foot Storativity: 0.0025 Velocity: 0.5 feet per day Monitor Well MW-WSP-5D was utilized for a pumping test to determine hydraulic properties of the surficial aquifer. This well is 4 inches in diameter with a screened intake set between 20 and 30 feet below grade. The well was pumped for approximately one and a half hours at a rate of 15 gallons per minute. Calculations were based on data collected from the pumping well and nearby observation wells. The data was plotted on a distance versus drawdown semi -log graph to aid in calculating transmissivity and storativity. (F) Groundwater in the water table aquifer flows to the north-northwest as determined by surveyed monitor wells and groundwater travels at a rate of about 0.5 feet per day based on pumping test data. (G) Included as Attachment 1 are the monitor well logs for the site. 3. INJECTANT INFORMATION A) The material to be injected is called WillClear+, which is a sodium lactate formulation made with carbohydrates and fermentation metabolites manufactured by JRW Bioremediation to stimulate anaerobic bioremediation of CVOC contaminants in groundwater. The MSDS is included to this form as Attachment 2. The sodium lactate material will be mixed with 50% water to lessen its viscosity to aid in the assimilation into the groundwater. B) The source of the water used to dilute the sodium lactate will be from an on -site "clean" well (Monitor Well MW-WSP-1D) which is located hydraulically upgradient of the former on - site CVOC source areas. As such, the same formation water will eventually be placed back into the ground with the food grade sodium lactate material. The log for Monitor Well MW- WSP-11D is included in Attachment 1 and a groundwater sample analysis are summarized in Table 2 attached to this form. C) The sodium lactate material was selected since it is food grade material and it stimulates anaerobic bioremediation of the CVOC impacted groundwater. The sodium lactate material lasts for a short term and does not by itself break down into harmful constituents. As indicated on the form, we propose to inject 3,575 gallons of sodium lactate diluted with 3,575 gallons of clean water obtained from an upgradient monitor well. The amount of injectant proposed to be used is based on the area of the site and the depth between the water table and the underlying confining unit referenced in Section 2 above. A calculation sheet provided by JRW Bioremediation and included as Attachment 3 shows how the injectant amount was derived. D) The reductive dechlorination process being evaluated would involve the stimulation of anaerobic bioremediation of the CVOCs by injecting food -grade sodium lactate into the affected areas of the shallow overburden water table. The process works by stimulating microbial growth and development, creating an anaerobic groundwater treatment zone, and generating hydrogen through fermentation reactions which support the proliferation of reducing bacterial colonies capable of dechlorinating PCE and other associated CVOC daughter by-products. The CVOC reducing bacteria destroy the CVOCs by replacing chlorine atoms within the solvent molecule with hydrogen atoms. E) The sodium lactate material becomes consumed by naturally occurring bacteria. F) The lactate injections will travel with groundwater flow to the northwest. As discussed above line C), the sodium lactate is quickly consumed in the groundwater by naturally occurring bacteria. 4. INJECTION PROCEDURE A) The Sodium lactate diluted with 50% water quantity to be injected during the initial event is proposed to be 7,150 gallons. We are proposing to inject the aforementioned quantity of sodium lactate through 6 injection wells and 25 soil borings advanced using a geoprobe rig. The work will take approximately two weeks to complete. The attached figure 5 shows the injection amounts per injection well and soil boring, and the locations, respectively and Table 3 summarizes the injection quantities. Subsequent injection events would be conducted in about six months with lesser injection quantities to maintain the anaerobic environment in the groundwater to sustain the bioremediation of the CVOC impacted groundwater. We are asking for a five year allowance on injections for this site. The timing for subsequent follow up injection events will vary based on the effectiveness of the initial event. B) The average injection pressure will be about 30 psi. C) Same as A) above. 5. FRACTURING PLAN Not applicable to our application. 6. WELL CONSTRUCTION DETAILS A) There are 6 existing wells to be utilized for the injection of the sodium lactate. B) There are 25 proposed soil borings to be advanced using a geoprobe rig. Each soil boring location will have an injection depth of 15 and 30 feet below grade. C) The injection wells are existing. D) The injection wells consist of PVC piping material and the depths are summarized on the attached table 1. E) See Table 1 and the well logs included in Attachment 1. F) See well logs included in Attachment 1. G) The injection wells are permanent, the soil borings are temporary direct push. H) See well construction logs included in Attachment 1. 7. MONITORING PLAN A) The target contaminants include tetrachloroethylene, trichloroethylene, cis-1,2- dichloroethylene, trans-1,2-dichloroethylene and vinyl chloride. B) We intend on collecting post -injection groundwater samples for analyses of volatile organic compounds, metals, biological organic demand, chemical organic demand, sulfate, nitrite/nitrate, gases (methane, ethane, and ethene), total organic carbon, and alkalinity. Field parameters will be collected for dissolved oxygen, oxidation-reduction potential, pH, temperature and conductivity. C) Figure 4 shows the locations of the monitor wells relative to the site to be utilized for monitoring the injection progress. D) To monitor the effectiveness of the injection work, the 14 monitor wells listed below will be sampled on a quarterly basis following the initial round of injection.
