Dear Mr. Price:
You have r approval, in accordance with the attached conditions, to place fill in 48 linear feet of unnamed tributaries to the
Lockwood Folly ver in order to construct road crossings accessing your property in Brunswick County, as described in your application
received by the D ision of Water Quality on June 12, 2001. After reviewing your application, we have determined that this fill is covered
by General ter Quality Certification Number 3289 or 3287, which can be viewed on our web site at
htt ://h2o.enr.stat .nc.us/ncwetlands. This Certification allows you to use Nationwide Permit Number 14 or 39 when it is issued by the
U.S. Army Corps f Engineers. In addition, you should get any other federal, state or local permits before you go ahead with your project
including (but not mited to) Sediment and Erosion Control, Non-Discharge and Water Supply Watershed regulations. Also this approval
will expire when t accompanying 404 permit expires unless otherwise specified in the General Certification.
This approv is only valid for the purpose and design that you described in your application. If you change your project, you must
notify us in writing nd you may be required to send us a new application for a new certification. If the property is sold, the new owner
must be given a c py of the Certification and approval letter and is thereby responsible for complying with all conditions. If total wetland
fills for this project now or in the future) exceed one acre, compensatory mitigation may be required as described in 15A NCAC 2H .0506
(h). For this appr al to be valid, you must follow the conditions listed in the attached certification. as well as the additional conditions
listed below:
1. Ro side ditches in the vicinity of the road crossings must be sloped back and vegetated to minimize sedimentation into
the tream beds, and this vegetation must be maintained in perpetuity;
If you do not ccept any of the conditions of this certification, you may ask for an adjudicatory hearing. You must act within 60 days
of the date that yo receive this letter. To ask for a hearing, send a written petition which conforms to Chapter 150B of the North Carolina
General Statutes t the Office of Administrative Hearings, P.O. Box 27447, Raleigh, N.C. 27611-7447. This certification and its conditions
are final and bindi unless you ask for a hearing.

PROJECT DESCRIPTION
This project consists of the construction of a single-family
residence and an access driveway approximatle 1100' in length.
The driveway crosses a single stream at two locations where corrugated
metal pipes and fill material have been installed to create the
crossings. The stream flows to the south, entering an adjoining
parcel of land, and then enters the Lockwood Folly River, where the
river is classified as SB waters. The area cleared for the drive-
way varies in width from about 15' to over 40'. The northern 500'
of the driveway crosses a terrain with up to 15' of relief and slopes
up to 16% in highly erodable sandy soils (Baymeade Fine SAnds and
Murville Mucky Fine Sands). The southern 600' of the driveway is on
an elevated, sandy upland with slopes less than 4% (Baymeade Fine Sands).
The area cleared for the homesite consists of approximately 0.3
acre. The home is setback 95' from the river.
