Division of Water Resources' Central and Regional staff has reviewed the application package
received December 20, 2019. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on March 23,
2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through C, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

A. Property Ownership Documentation 115A NCAC 02T .0116[clI:
It is stated on plan sheet WW-2 and WW-3 that the supply and return lines of the drip disposal field
will have to run underneath Brogden Road, a public right of way maintained by the North Carolina
Department of Transportation (NCDOT). Private utility crossings on a public right of way require
approval from the NCDOT before construction is allowed. Documentation shall be submitted
demonstrating that the Applicant's proposed right of way crossing listed on plan sheet WW-2 and
WW-3 has been approved by NCDOT.
B. Enyineerina Plans:
1. The submitted engineering plans did not provide sufficient information regarding the proposed
utility crossing of Brogden Road. Plan and profile views of this crossing shall be submitted for
review detailing alignment, depth, and appurtenances in accordance with 15A NCAC 02T
.0604(c)(1).
C. Specifications:
1. The methods of construction for how the installation of the supply and return lines underneath
Brogden Road without disturbing the right of way were not submitted for approval in this
application. Specifications dealing with the installation of lines in this case shall be submitted for
review in accordance with 15A NCAC 02T .0604(c)(2). This amended package shall include any
technical standards/specifications that are required in order to obtain NCDOT's approval.
