A review of the November 2019 and December 2019 Discharge Monitoring Reports (DMRs) for
the subject facility revealed the violation(s) indicated below:

Noncompliance Notification: (9) The permittee shall report by telephone to either the central
office or the appropriate regional office of the Division as soon as possible, but no later than 24
hours or on the next working day following the occurrence of:

(a) Any process unit failure, due to known or unknown reasons, that render the facility
incapable of adequate wastewater treatment such as mechanical or electrical failures of
pumps, aerators, compressors, etc.
This information was also conveyed during the Compliance Inspection site visit of July 16, 2018,
and recorded in the Compliance Inspection Report dated July 27, 2018.
Please be advised that this letter is a Notice of Violation. You are requested to respond to this
Notice in writing within fifteen (15) days of receipt indicating your actions taken to resolve the
non-compliance issues and prevent them in the future.
Failure to comply with conditions in a permit may result in a recommendation of enforcement
action, to the Director of the Division of Water Resources, who may issue a civil penalty
assessment of not more than twenty-five thousand ($25,000) dollars against any "person" who
violates or fails to act in accordance with the terms, conditions, or requirements of a permit
under authority of G.S. 143-215.6A.
