Dear Mr. Rhea:
In accordance with your request for Temporary Offsite Storage of Residuals, and all subsequent information
provided, the Winston-Salem Regional Office (WSRO) is forwarding this approval to Louisiana Pacific
Corp. to begin use of the time -limited operation of a temporary offsite storage structure.
This approval shall be effective for a period of 60 contiguous days from the effective date of this
approval of February 15, 2020 and shall be subject to the conditions and limitations as specified in
Permit No. WQ0000461. Temporary storage shall be conducted in accordance with the plans and the
information provided with the request, dated February 6, 2020. This approval is site specific to Mathis
Properties, LLC (615 Mathis Farm Rd. Roaring River, NC 28669) as proposed in the information included
with the request. This approval shall become voidable unless the agreements between Louisiana Pacific
Corp. and Mathis Properties, LLC for temporary storage of residuals are in full force and effect.
