I was a little confused on what I needed to attach. All the city inspector said was that we needed to file the NOT and he
would close out our permit. I have attached the signed NOT Certification Form. I did mail a hard copy yesterday as
well. Please let me know if you need anything else.

Hello,
We received an NOT request for this COC No. NCC190445 for Cracker Barrel in Winston-Salem. However, a completed
eNOT Certification form was not uploaded (attached) — instead just another copy of the email from the City was
uploaded.
Also, the email from the City did not indicate any kind of close-out inspection results or date of inspection. It just gave
instructions about what to do when the project was closed. Is there any other documentation from the City about a
closeout inspection taking place on 2/17?
If you can email this additional information/required form, I can process the NOT.
